An exciting opportunity has arisen for the position of Reablement Occupational Therapist within Glasgow HSCP Care at Home Assessment Team. Reporting to the Team Leader (Reablement), you will be responsible for the provision of Occupational Therapy assessment and intervention for service users accessing Care at Home services. Role – Occupational Therapist Location – Glasgow Salary Range – £41,486.53 - £48,166.60 per annum (core salary, which also includes Working Contents and Demand payment) Hours and Shift Pattern – 35 hrs per week (8:45am to 4:45pm Monday to Thursday, 8:45am to 3:55pm Friday). This is a hybrid working role with a requirement to travel to support frontline teams across the city. The main duties and responsibilities will include the screening and management of referrals into Glasgow’s in-house HSCP Care Services. In addition, you will be responsible for the assessment, planned intervention and review of individuals, including the provision of equipment and minor adaptations. You will be expected to manage a diverse caseload with high demand and throughput. The focus of your intervention will be on functional assessment and goal setting in order to maximise and maintain independence for the service users. You will join an established and supportive team and work in partnership with the care services operations team, home carers, social care workers and the wider multi-disciplinary HSCP teams. You will be responsible for completing assessments and tailored intervention to ensure that service users’ needs are met and in line with current policy and procedures. To succeed in this role, you must be able to demonstrate working knowledge of relevant legislation, policies and procedures, in line with the Maximising Independence and Reablement Model in Glasgow. The successful applicant will have a flexible approach to service delivery and demonstrate the ability to communicate effectively with internal and external customers and other agencies. You will be required to work on your own initiative as well as part of a team and possession of a UK Driving Licence is desirable. There is the opportunity for a flexible working pattern and hybrid working across 35 hours Monday to Friday within core hours. Applicants must possess a Degree / Diploma in Occupational Therapy and be HCPC registered. We would also welcome applications from Occupational Therapy students who are qualifying in this years cohort.
