The Peer Specialist functions as an interdisciplinary team member, assisting physicians and other professional/non-professional personnel in a rehabilitation treatment program. Peer specialists perform a variety of therapeutic and supportive tasks that include assisting Veterans in articulating their goals for recovery and personal wellness, learning, and practicing new skills, helping them monitor their progress, assisting them in their treatment, modeling effective coping techniques and self-help strategies based on the peer specialist's own recovery experience, and supporting them in advocating for themselves to obtain effective services.

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The Senior Social Worker is primarily responsible for providing psychosocial and mental health assessment, diagnosis, treatment planning/execution, care management, case management, crisis intervention and evidence-based psychotherapy services to Veterans, their family members and caregivers in within Acute Medicine-Surgery units.

Duties for this position include but are not limited to:

• Receives referrals from a variety of sources to include triage, consults, walk-in visits, phone calls, and warm hand-offs from internal/external stakeholders. The Senior Social Worker is required to utilize available data sources to proactively identify patients for screening/triage, assessment, treatment planning, case management and care coordination.
• Provides crisis intervention services, seeking to address the cause as well as presenting complaint, coordinates family conferences and serves as a liaison to family members/caregivers. During each encounter, the PACT Care Manager RN focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, performing procedures, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient to navigate the health care system, and evaluating patient outcomes. The PACT Care Manager RN, acting as a partner and advisor, assists and supports patients and families to optimally manage their health care, respecting their culture and values, individual needs, support in self-management, health goals and treatment preferences. They facilitate continuity of care using the nursing process, multidisciplinary collaboration, and coordination of appropriate health care services and community resources across the care continuum.
