· To perform advanced Occupational Therapy assessments of patients with diverse presentations and complex physical and psychological conditions. Use advanced clinical reasoning skills to develop and deliver individualised treatment programmes.
To evaluate patient progress, reflect, analyse and reassess intervention and alter treatment programmes, as appropriate. Clearlycommunicating any changes in care plan or treatment approach to other team members to ensure consistency of approach and enhance recovery.
· To maintain up to date and timely clinical records in accordance with service and professional standards and to access and update patient information as required.
· To hold clinical responsibility for a designated caseload, seeing patients in the most appropriate community setting.
· To provide leadership and training for junior members of staff and support workers within the team and to supervise occupational therapy students on placement.
To work as an autonomous practitioner within the multidisciplinary team.
To undertake relevant competency training, specific to the service area and appropriate to the role banding. This may involve medicines administration and basic wound care.
You will work as part of multi disciplinary team, alongside physiotherapists, nurses, support workers, associate practitioners and consultants.
You will be responsible for the assessment of patients allocated to you, and for the provision of their treatment plan and subsequent interventions, You will be required to complete joint visit with other members of the team. To be able to feedback to the cordintors in the serivce a clear plan for each patient.
You will need to complete on ward referrals to other services, and ensure and make recommendations for the provision of equipment and or adaptations if deemed appropriate.
You will also be involved in the assessment of patients with cognitive impairment, and dementia and will expected following appropriate training to be able to take patients observations, and complete clear concise and appropriate records.
You assessments will take place in a number of different settings but predominantly in the patients own home.
