Our assessor will provide end to end structured support to our apprenticeship learners on the programme. They will ensure the most effective and up-to-date approaches are implemented to prepare learners to successfully progress in their Health and Social Care qualification.
Our apprenticeship program provides learners with hands-on training and theoretical knowledge in Health and Social Care, equipping them with essential skills for a successful career in the industry.

Your impact
• Ensure the highest training standards by adapting to learner needs.
• Cover a regional area and travel to learners' workplaces for assessments.
• Complete Individual Learning Plans (ILPs) and outline personalised learning pathways.
• Reference and evaluate evidence collected in the workplace to meet key skill requirements.
• Maintain accurate records of learner attendance and guided contact hours, ensuring compliance with audit requirements.
• Contribute to the planning, monitoring, and achievement of set performance targets.
• Arrange additional training through day release and provide in-house training support as needed.
• Keep detailed records of clients' achievements to meet the performance criteria established by relevant awarding bodies.

What are we looking for?
Essential
• Proven occupational experience and technical competence in Health and Social Care delivery.
• Full driving license and access to a vehicle.
• Willingness to undergo a DBS check (cost covered by the company) and EWC registration.
• Welsh speaking is desirable.

To be responsible for the delivery of practice nursing services to the practice population, working collaboratively with the rest of the practice team. The practice nurse is responsible for a number of clinical areas such as chronic disease management, vaccination and immunisation and general treatment room duties, as well as certain defined administrative duties. An exciting opportunity has arisen for permanent Band 5 Community nurses working within the South Rural Integrated Neighbourhood Team. You will be passionate, committed, innovative, enthusiastic, and highly motivated to delivering high quality care within a patient's home.

The South Rural INT is based at Hadleigh Health Centre and East Bergholt, providing an opportunity to work within a knowledgeable experienced team and wider MDT. You will be required to work across the South Rural INT and the wider community services to meet service requirements. The team consists of district & community nurses, assistant practitioners, generic workers, occupational therapists, physiotherapists, and social care workers. The team is led by a lead nurse and overall managed by an INT manager, supported by an administrator and care co-ordinator.

The team is proud of their holistic care approach and work shifts covering the operational hours of 08:00 to 18:00 over 7 days.

The team continues to change to support developments in the NHS which includes close working relationships with the local statutory, non-statutory and voluntary services. You will be expected to work autonomously and on behalf of the patients served in the local area. You should therefore be flexible, motivated, and willing to work within a changing environment.

Your duties would include wound care, leg ulcer management, pressure area care, palliative care, venepuncture, attending GP multi-disciplinary team meetings, PICC line care, male, female suprapubic catheterisation, ordering equipment, support and mentoring of students and involvement in link roles such as tissue viability.

Please note that Beacon House is a temporary location. This is likely to remain the teams base location for the next few years while building work is completed at the Hadleigh Health Centre. Once complete this role will be based at Hadleigh Health Centre.

Job Responsibilities: Provide day to day support, leadership, first line management and guidance for the practice team, ensuring an efficient and professional manner is maintained and to provide cover to the admin team, when needed. Managing patient records and IT systems as the Lead for Information management. Chair monthly safeguarding meetings alongside clinical chair. Prepare agendas for meetings and record minutes. Produce weekly rotas for team whilst taking into account all leave, training and any other issues which need to be included or may impact on the rotas. Ensuring that there is adequate cover and developing solutions to maintain safe staffing levels where necessary. Identify any training needs for team and arrange/provide necessary training. Coordinate the activities of the admin team daily and provide training with support from Management team to new and existing staff where appropriate. Ensure compliance with all company policies, procedures, and guidelines. Continually assess and evaluate systems and quality, recommending changes and improvements to the management team as appropriate Manage day to day issues that may arise within the team or with individuals. Regularly evaluate quality of individual and team performance Bi-monthly team meetings with all admin staff Support the annual appraisal process for admin staff. Administrative Duties: Typing of letters, memos, reports, minutes of meetings, etc. To ensure all information is up to date/current and communicate information with all admin staff, ensuring relevant changes are embedded. Amendment of processes and procedures in agreement with management team. To undertake a variety of administrative duties to assist in the smooth running of the practice including management of patient results etc. To support the practice management team in achieving QAIF standards and targets e.g. booking appointments, clinics. Computer data entry/data allocation and collation; processing and recording information in accordance with practice procedures. Responding to emails, telephone calls, letters and voicemail messages using initiative, giving information and taking due care in relation to GDPR & Data Protection Act. Collate and store any documentary evidence for audit purposes. Filing and storing records as required Answer incoming calls, transferring calls or dealing with patient requests appropriately. Scanning of hospital letters/patient information onto patients medical records as required
