This social worker is assigned to the HBPC Program and serves as an outpatient social worker in the Providence VA system.
Duties include, but are not limited to:
- Perform comprehensive initial clinical assessments and follow up assessments when clinically indicated or at a minimum annually, in-home assessments of the psychosocial needs and functioning of the patient, caregiver, and their support systems;
- Recognize psychosocial issues impacting the patient/caregiver's ability to achieve optimal outcomes and implementing interventions and provide social work case management to address psychosocial needs to support patient driven goals of care;
- Conduct psychosocial assessments, depression screenings and caregiver burden screenings within 30 days of HBPC admission to determine needs of veterans and family members and participate in the treatment planning process with other disciplines;
- Develop a psychosocial plan of care which may include individual/family counseling, long-term and advance care planning, non-pharmacological pain interventions, stress management, caregiver respite, psycho-educational programs, as well as grief and bereavement counseling;
- Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate;
- Provide individual, family and group counseling as appropriate, as well as provide consultation/education to veterans and families on community resources, advance directives, and VA benefits;
- Act as liaison between VA and community resources, to include marketing and public relation duties as needed;
- Provide case management to veterans and families, as needed, throughout the continuum of care;
- Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate;
- Maintain data and statistical compilations to comply with Joint Commission, VA and medical center policy and procedures;
- And other duties as assigned.
Work Schedule: Part-time, 8:00am - 4:30pm. Tour of duty to be determined.
