The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: a. To deliver a safe, effective, kind and and responsive nursing service to our patients, always recognising, and within, the confines of your own professional qualifications and capabilities. b. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice. c. To provide core primary care nursing services to patients, for example, phlebotomy, ECG, BP, cervical smears, suture removal etc d. To deliver opportunistic health promotion where appropriate for example healthy eating, smoking cessation, alcohol consumption etc e. Perform holistic assessment of patients attending for ear irrigation f. To implement vaccination programmes for adults and children g. Provide wound care (ulcer/Doppler, etc.) to patients and implement wound care in line with current evidence based guidelines h. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice i. Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions j. To provide NHS travel medicine services k. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing l. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health m. As required and as appropriate to skill set to diagnose and manage acute and chronic conditions n. To maintain accurate clinical records in conjunction with extant legislation o. To ensure compliance with the practice chaperone policy p. To assist GPs with minor surgery or other specialist clinics as required q. To prioritise health issues and intervene appropriately r. To support the team in dealing with clinical emergencies, including recognising and manage anaphylaxis and able to perform Cardio-pulmonary resuscitation all according to current UK guidelines s. To recognise, assess and refer patients presenting with mental health needs t. Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse. u. To support the clinical team with all safeguarding matters, in accordance with local and national policies v. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) w. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) x. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Secondary key responsibilities In addition to the primary key responsibilities, the Practice Nurse may be requested to: a. Participate in review of nursing protocols as directed by the management team. b. Participate in local initiatives to enhance service delivery and patient care c. Support and participate in shared learning within the practice in order to improve patient care d. Contribute to the assessment of service needs and contribute a nursing perspective to the practice development plan e. Continually review clinical practices, responding to national policies and initiatives where appropriate f. Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA) g. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation
