Clinical Duties:

To work as a Band 6 Community Mental Health Practitioner in the Dementia Intensive Support Service in Mid Essex which is a team that is part of the Older Adult Community Mental Health, Dementia and Frailty Service. The post holder will be responsible for providing rapid assessments for people living with a dementia or a suspected dementia that are in a Crisis situation that requires home treatment to avoid a hospital admission.

The post holder will also be aligned to support a newly formingFunctional Intensive Response and Support Team (FIRST), which will provide assessment, support and appropriate care planning to prevent unnecessary hospital admission and support early discharge when the individual is admitted.
