As a Mental Health Practitioner you will be providing specialist mental healthcare to offenders and working as part of the Mental Health In-Reach Team, plus wider mental health services. You will be required to work in a psychologically minded way with offenders in achieving their agreed goals and quality health outcomes.

Our Mental Health Team provide specialist interventions at primary and secondary care level including; referral management, screening assessment, triage, evidence-based interventions, care planning and risk assessing, plus one-to-one and group-work facilitation.

Our Mental Health Practitioners manage a mixed and challenging caseload and are required to perform robust assessment, screening and interventions to offenders with learning disabilities and mental health conditions. You will contribute to alternatives to inpatient admission and assisting with early discharge through the implementation of high intensity interventions and complex case management arrangements.

One of our key aims is to ensure continuity of care for an offender on ordinary location and reduce the length of stay, both in relation to prison inpatient services and external NHS/independent in-patient services. You will work closely with community mental health teams to ensure appropriate sharing of information, continuity of care and the Care Programme Approach for all offenders as necessary.

Frenchay Brain Injury Rehabilitation Centre, part of the Active Care Group, is a modern independent hospital providing an inpatient Level 1 intensive brain injury rehabilitation service (29 NHS England funded beds) for the South West of England and a further 23 slower-stream neuro-rehabilitation beds (Level 2). The HCA will be involved primarily in undertaking the new patient health checks, over 75 health checks, urinalysis, ECG recording, spirometry, lifestyle advice & weight management, administering authorised vaccination chaperoning patients, maintaining hygiene standards, maintenance of equipment and the recording of refrigerator temperature on a daily basis, ordering of stock and re-stocking GPs and Nurses treatment rooms, maintaining the emergency drug checklist. The HCA will also be responsible for documenting his/her patient consultations on the EMIS clinical information system in General Practice. He/she will maintain patient confidentiality and will work under the direct supervision of the Senior Practice Nurse & lead GP and within Practice Guidelines and Protocols at all times. Clinical Duties To assist in restocking clinical area with supplies and health promotion materials as agreed with GPs/Nursing Team and to restock all rooms as appropriate. Monitor emergency drugs check list To assist in general nursing duties, under the supervision of the Practice Nursing Team, i.e. Urine testing, weight/height, blood pressure measurements, ECG recording, spirometry, peak flow measurements smoking cessation advice and any other duties that may be assigned as necessary. To ensure that drug/vaccine refrigerators are at the correct temperature on a daily basis and maintain the Vaccine Cold Chain documented record according to Practice Guidelines, in the absence of designated Nurse. To act as a chaperone as required to GPs and Practice Nurses. To participate in the induction programme of staff new to the practice To receive and document telephone messages as and when necessary and communicate to other members of the clinical team as appropriate. To maintain health information notice board in line with national/local health promotion campaigns. To undertake appropriate formal training as required by the Practice. Administer B12, Influenza, Shingles, RSV & Pneumococcal vaccines under PSD. Carry out home visits to perform health checks on the housebound and frail elderly population. Support joint home visits with the nursing team for the delivery of vaccination programmes. General Duties To function as an integral part of the Primary Health Care Team. To attend relevant clinical meetings as required. To have knowledge of and act at all times within the boundaries of Infection Control & Health & Safety legislation & Data Protection acts. To report immediately to a member of the clinical staff or the Practice Manager any acts of omission or error to safeguard the interest of the patient. Undertake ECGs and Audiograms New patient health checks for newly registered patients NHS Health Checks Giving injections to the practice population according to protocols Ensure adequate infection control procedures in treatment and consulting rooms. Ordering medical supplies and checking/recording and storage of supplies on receipt Inputting of information onto EMIS Ensure the safe storage, recording, rotation and disposal of vaccines. Have a broad knowledge of wound types and methods of management, i.e., burns, lacerations, sebaceous cysts/drainage and insertion of wick dressings, postoperative wound care, and suture and clip removal, minor skin injuries; i.e. grazes and bruises. Refer patients to medical colleagues for a review when necessary. Leg ulcer dressings and management - knowledge of ulcer dressings, ability to assist with these dressings in the absence of a specialist nurse. Minor Surgery / Coil / Implanon contraceptive implant fitting - prepare necessary equipment, support the patient and assist the GP during the procedure. Ensure asepsis during procedure. Supporting new members of staff / nursing and medical students. Taking skin scrapings & nail clippings and complete relevant paperwork for fungal skin/nail investigations. Pregnancy testing. Helicobacter testing as per test kit/instructions as issued. Administration of medicines via the injectable route, whether routine or emergency based. Administer and support patients undertaking oxygen therapy or nebuliser treatments. Assess the patient presenting with a minor injury and implement the most appropriate course of action, refer to a GP / refer to A&E if necessary. Ensure correct data entry in patients notes for any minor injury within the past 48hrs, adhering to the minor injury guidelines. Chronic disease management if qualified and approved by Practice then responsible for running clinics autonomously with the ability to instigate basic diagnostic tests, developing and maintaining the practice register and working actively with the lead GP in achievement of targets etc. Caseload management, aiming to identify those people with a significant risk but who have not developed symptoms and offer advice and treatment. Source and supply up to date education material. Educate and encourage patients in self-management of their condition. Any other duties as directed by the Managers or GPs.
