JOB SUMMARY To provide specialist pharmaceutical support to the 5 Practices of Severnvale Primary Care Network. To liaise with patients, clinical and non-clinical staff, practice, hospital and community pharmacists and patient's carers to improve the pharmaceutical care of patients. To consult with patients. To support the Senior Clinical Pharmacist and fellow Clinical Pharmacists role to maximise support to all clinical and non-clinical practice staff to improve patient care in relation to all aspects of medicines and their impact. MAJOR DUTIES AND RESPONSIBILITIES To carry out medication reviews and structured medication reviews for patients both in the surgery, in care homes or for housebound patients - with single or multiple medical problems where medicines optimisation is required; reviewing the need for each medicine and providing recommendations for other clinicians on prescribing. To consult with patients, by telephone and face to face according to competency. This may include patients with long term conditions, e.g. hypertension, CVD, diabetes as well as medication reviews. To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge. To ensure patients have appropriate monitoring tests in place when required. The Medical Secretaries and Admin team provides support to our clinical team 11,500 patients. You will be liaising on sensitive medical information, so a caring customer service communication style is essential. You will also facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies. This is a well-established joint post between Bath University and NBT which is equally split between both roles. The post holder will be responsible for developing, delivering, and assessing practice-based learning at an undergraduate levelandbe part of the Pre-Op Surgery pharmacy team providing a clinical service to the specialty.

This joint appointment will involve working collaboratively with other practice educators to design, deliver and assess practice based learning elements of the pharmacy undergraduate degree course including our very successful Clinical Learning in Practice (CLIP) model. The successful individual would be required to cover a range of face to face teaching activities and elements of assessment. There is also a requirement to provide pastoral support to individual students.

Whilst working within the hospital you will be part of the Pre-Op surgery team, providing cover to the clinic on a sessional basis and supporting the clinical management of surgical patients at NBT.

The role has a major focus on teaching and potential candidates must therefore have excellent presentation and communication skills, have completed a clinical diploma, and be able to work both at the University in Bath and Southmead hospital in Bristol.

Design and deliver high quality teaching which is informed by significant professional experience gained via practice, research/study and teaching experience, to undergraduate students on the new MPharm programme - to include lectures, workshops, applied pharmacy practice skills sessions and supervision of dissertations or projects, assessment and marking and pastoral care of students, in line with Department and University policy. Practice Educator Role
1. Pre-Op Surgery Pharmacist
1. Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Anaesthetics, Surgery, Critical Care and Renal (ASCR) Division to achieve a high standard of efficiency and effectiveness.
2. Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Pre-Operative Assessment Clinics and managing the pharmaceutical care in the per and peri-operative period.
3. Clinically screen prescriptions to ensure the safe, efficient, evidence-based and rational use of medicines in line with the BNSSG Formulary and departmental policies so that risks to patients areminimised.

Clinical: 1. To have responsibility and accountability for the management of own clinical caseload, including referrals, management and discharge decisions, within own level of competence and with appropriate clinical support. 2.
