The post holder will be responsible for: Coordinating the Multi-disciplinary team (MDT) eligibility assessment of an individuals needs in regard to eligibility for CHC Identifying and securing the involvement of the MDT which will assess the individuals needs and will then use this information to complete the Decision Support Tool (DST) supporting MDT members to understand the role they will need to undertake in participating in a multidisciplinary assessment and completing the DST. Facilitating Continuing Healthcare assessments for patients across all aspects of care. This necessitates a high level of assessment skills and requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care professionals, patients' carers and family members. The post holder will require Registered Nurse qualification. Supporting the person (and those who may be representing them) to play a full role in the eligibility consideration process, including ensuring that they understand the process, they have access to advocacy or other support where required, and organising the overall process in a manner that maximises their ability to participate. Providing highly specialist advice in relation to the National Framework for Continuing Healthcare eligibility in complex situations, necessitating a high level of interpersonal and communication skills. This includes care procurement incorporating personal health budgets, and ongoing care management and reviews of eligible individuals and providing information in a timely manner. Ensuring that there is a clear timetable for the decision-making process, and DST processes are completed in accordance with the requirements in the National Framework. Acting as an impartial resource to the MDT and the individual on any policy or procedure questions that arise Ensuring that the MDTs recommendation on eligibility is sent for ratification through the relevant local decision-making processes in a timely manner Ensuring timely "End of life" care procurement and ongoing management for individual eligible for the Fast Track pathway within the CHC service. Completing CHC / FNC / Joint Funded / Fast track reviews in a timely manner in line with the national framework. As a care manager, will co-ordinate and undertake ongoing care package reviews for individuals and liaise with the multi-disciplinary team to ensure that the package is able to meet needs safely and in accordance with Sheffields Commissioning Care Principles for those who are eligible. Ability to communicate complex information and handle sensitive conversations in a way which supports understanding of the recipient. Providing relevant continuing healthcare advice to secondary care, primary care, nursing homes etc. in relation to the continuing healthcare process. Developing and maintaining effective and appropriate communication networks with health and social care professionals and act as a resource to them. Interpreting national and regional guidance relating to the implementation of the National Framework and subsequent developments within the CHC service. Exercising professional judgement as necessary when communicating information on behalf of the CHC service and application of the associated criteria. Working with relevant health professionals, CHC colleagues, business support staff, senior leads and other support staff within the CHC team to ensure delivery of quality premiums and products required. Delivering specialist formal and non-formal education programs to Community and hospital staff including nursing/residential homes, GPs, community nursing and therapy staff relating to the assessment and management of CHC patient groups in line with the latest research, evidence and DHSC/NICE guidance. It is expected that care managers will operate as part of a wider team, providing cover for each others caseloads during periods of leave or absence and an ongoing inter-team support network, ensuring the needs of the whole patient group are met with a consistent and best practice orientated approach. The post holder will hold and manage an active case load, providing care management for individuals who are eligible for full NHS funding in order to meet their identified care needs. The post holder will be involved in identifying appropriate care placements and setting up a package of care delivery to meet identified assessed care needs. They will present these to the ICB Sheffield resource panel to inform and advise how an individuals identified care needs can be met. The post holder will represent individuals who are care managed, attending MDT meetings, at times instigating and leading on these meetings, around decision making processes, such as assessment of mental capacity and best interest decision making. The post holder will take an active role in risk management and safeguarding of all individuals on their active caseload, liaising with other professionals and attending meetings as and when required. The post holder will be required to be an autonomous practitioner, able to work independently and routinely travel significant distances both within and out of Sheffield as the needs of the case load require. As an experienced member of the Continuing healthcare team the post holder will be able to provide guidance, training and support to other team members. The Sheffield Continuing Healthcare team can take students; the post holder will be expected to work with and support these students and act as a mentor to them.
