Job Summary:
We are seeking an experienced and passionate Nurse Care Manager to join our dynamic team in providing high-quality care to clients with complex medical conditions, mental health, and substance abuse disorders. The successful candidate will work collaboratively with theCPRC team to educate and support clients, develop wellness initiatives, and facilitate health education groups. Key Responsibilities:
- Collaborate with the interdisciplinary team to create individualized treatment plans for clients with co-occurring chronic diseases
- Develop and implement wellness and prevention initiatives to promote client health and wellbeing
- Facilitate health education groups, utilizing expert knowledge of complex medical conditions
- Manage hospital and medical provider admissions and discharges for Healthcare Home patients
- Train patients on medical diagnoses, treatments, and medications
- Implement DMH Net Health technology programs and initiatives, such as CyberAccess and metabolic screening
- Monitor and report performance measures and outcomes using advanced information systems designed for nurses
- Demonstrate excellent written and verbal communication skills
- Utilize critical thinking and nursing process to make informed decisions
Requirements and Skills:
- Active RN or LPN license
- Direct patient care experience preferred
- Strong understanding of chronic disease processes in children and adults
- Excellent communication and interpersonal skills
- Ability to adapt to new technologies and implement innovative systems
- Critical thinking and problem-solving skills
- Passion for delivering exceptional patient care
