Duties of the RN HCBS Case Manager include, but are not limited to:

• Uses knowledge of the home and community-based care program and VA Program to organize delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning.
• Applies nursing clinical knowledge/judgement to promote staff involvement in planning, decision-making, and evaluating Veteran needs and outcomes
• Coordinates care for Veterans receiving both internal VA and community-based home and community-based services including but not limited to skilled home care, non-skilled homecare, adult day health care and respite care. This includes close collaboration with other connected internal VA programs including but not limited to caregiver support program, inpatient care, outpatient services, Veteran Directed Care, Home Based Primary Care, Bowel and Bladder and other programs.
• Evaluates, systematically, current practice and formulates outcomes for groups of patients, and/or organizational processes within area of expertise.-Works with inter professional team members at VISN and facility levels to ensure services and eligibility determinations are delivered objectively with strong, consistent reliability with documentation and communication of outcomes in a direct and coordinated manner.
• Conducts comprehensive clinical review of patient records to identify the presence of Veteran medical necessity for services.
• Evaluates and documents Veteran progress and changing needs.
• Executes position responsibilities that demonstrate leadership, experiences, and creating approaches to management of complex patient care.
• Reviews and receives community care consults.

Major duties include, but are not limited to:

• Incumbent is responsible to the Supervisor Laundry Operations Specialist for assuring that the work assignments of the other employees of the section are carried out.
• Distributes and balances the workload among employees in accordance with established work flow or job specialization. Assures timely accomplishment of the assigned workload.
• Monitors progress of work and makes day to day adjustments in accordance with established priorities. Obtains assistance from the Supervisor Laundry Operations Specialist as needed.
• Incumbent maintains records of work accomplishments and prepares production reports as required.

The Community Care Referral Case Manager (RCM) Registered Nurse duties include, but are not limited to, the following:

• Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
• Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.

The Nurse Manager of Fort Meade VA Community Based Outpatient Clinic is a Registered Nurse with 24/7 accountability for nursing care by licensed and unlicensed staff. S/he is responsible for the day-to-day operations ensuring patient and staff safety in compliance with VHA, VISN, and VAMHCS policies/procedures/nursing protocols, accepted best practices, and professional standards of care. S/he evaluates performance, provides annual competency training, and continuing education for direct reports. The Nurse Manager performs timecard certification, verifying staff schedules and hours worked. S/he practices fiscal responsibility within the budgetary guidelines. S/he is responsible for the implementation of the organization's Vision, Mission, and Values. The Nurse Manager consults with and reports to the Chief Nurse of Ambulatory and Emergency Care Clinical Center. The Nurse Manager is an essential leader in the clinic overseeing the clinic operations as well as ensuring safe and quality patient care and excellent customer service. S/he manages and evaluates the nursing staff who directly impact the multiple quality metrics.

• Assists the Fitness Facility Manager in the overall management and operation of a fitness facility; leads and or supervises the operation during second shift of program operations and in the absence of the Fitness Manager.
• Assist in the publicizing sports facilities, programs and events by drafting articles for local newspapers and as instructed by supervisor coordinates with marketing for posters, flyers and other promotional material.
• Oversees the inventory of property; fitness equipment condition; tracks maintenance and repair or replacement as appropriate.

HOW YOU'LL WORK:
You’ll assume ultimate accountability and leadership for the assessment of the patient condition and plan of care. You will case manage and provide clinical care under the direction of the physician. You’ll educate patients and their family members and ensures the safety of the patient. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. 

MAJOR AREAS OF RESPONSIBILITY: 

• Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change.

• Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.

• Supervision: Oversee and supervise total care of patient provided by nurse aides and LPNs

• Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.

• Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.

• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.  Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.

• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.

• Operations: Attend in-service training and mandatory agency meetings
