About the Role

We are excited to be able to advertise this role for a senior academic in health psychology with considerable experience in higher education. You will contribute to the academic leadership and delivery of our MSc Health Psychology programmes and to the research profile of the School of Psychology, including applied health psychology research.

As we grow our health psychology provision, this role will include oversight, leadership and development of our campus and online programmes, working with the Head of Discipline, senior academics and the wider teaching team to ensure continuation of excellent student experience and outcomes.

The role will include leading and supporting our marketing events (e.g., open days) which occasionally take place into evenings and at weekends.

The School of Psychology has an active research agenda, and you will be expected to engage with and support financially sustainable research and/or other appropriate academic activity.

There will be opportunity for working from home as agreed with the Head of Discipline. We do support hybrid working as business needs permit. There is currently no requirement for line management in this role. We are advertising this as a full-time position but would be happy to consider candidates who would prefer part time.

About You

To be successful in this role you should have experience of curriculum development and design, and significant teaching experience in health psychology. You should already hold FHEA and may hold SFHEA (or be willing to work towards this). You should have a track record of high quality and impactful research and be able to demonstrate engagement with industry in the development of research. You should also be able to demonstrate a track record in securing external funding. Excellence in communication, team and collaborative working are essential, both internally and externally.

Being a HCPC registered (or eligible) Practitioner Psychologist (health domain) is desirable for this role but not essential.

The post holder will be responsible for the day-to-day management and development of the pharmacists and technicians and the distribution of the pharmacist and technician resource within a Neighbourhood, liaising with the member practices and the PCN Pharmacy lead. Key areas of Focus Patient-facing medication reviews of patients with polypharmacy especially for older people, people in care homes and those with multiple co- morbidities. Leadership to practices and pharmacy team on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework. Working as part of a multi-disciplinary team to manage repeat prescription authorisations and reauthorisation, acute prescription requests, management of medicines on transfer of care and systems for safer prescribing. Providing expertise in clinical medicines management while addressing both the public health and social care needs of patients in the GP practice(s). Day to day management of the PCN pharmacists and technicians in the neighbourhood and agreeing allocation of the resource with member practices. Working across and assisting with both practice and PCN requirements Key duties and responsibilities 1. Patient facing medicines support Hold clinics for patients requiring medication reviews i.e. a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine taking. Provide a telephone support for patients with questions, queries and concerns about their medicines. 2. Structured Medication Reviews (SMRs) Undertake structured medication reviews with patients with multimorbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests. 3. Risk stratification Design, development, and implementation of computer searches to identify cohorts of patients at high risk of harm from medicines. Responsibility for management of risk stratification tools on behalf of the practice. Working with patients and the primary care team to minimise risks through medicines optimisation. 4. Longterm condition clinics See patients in multimorbidity clinics and in partnership with primary healthcare colleagues and implement improvements to patients medicines, including deprescribing. Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking. 5. Unplanned hospital admissions Devise and implement practice searches to identify cohorts of patients most likely to be at risk of an unplanned admission and readmissions from medicines. Work with case managers, multidisciplinary (health and social care) review teams, hospital colleagues and virtual ward teams to manage medicines-related risk for readmission and patient harm. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. 6. Care home structured medication reviews Manage own caseload of care home residents. Undertake clinical medication reviews with patients and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests. Work with care home staff to improve safety of medicines ordering and administration. 7. Repeat prescribing Produce and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates; make necessary changes as an independent prescriber, and ensure patients are booked in for necessary monitoring tests where required. 8. Medicines safety and quality improvement Identify and provide leadership on areas of prescribing requiring improvement. Conduct own audits and improvement projects and work with colleagues. Service development Develop and manage new services that are built around new medicines or NICE guidance, where new medicine/recommendations allow the development of a new care pathway (e.g. new oral anticoagulants for stroke prevention in atrial fibrillation). 12. Care Quality Commission Provide leadership to the practice manager and GPs to ensure the practice is compliant with CQC standards where medicines are involved. 13. Population and Public Health To devise and manage population and public health campaigns to run within the network. To provide specialist knowledge on immunisation. 14. Cost saving programmes Make recommendations for, and manage pharmacy technicians to, make changes to medicines (switches) designed to save on medicine costs where a medicine or product with lower acquisition cost is now available 15. Medicine information to practice staff and patients Answers all medicinerelated enquiries from GPs, other practice staff and patients with queries about medicines. Providing follow up for patients to monitor the effect of any changes. 16. Information management Analyse, interpret and present medicines data to highlight issues and risks to support decision making. 17. Training Provide education and training to primary healthcare team on therapeutics and medicines optimisation. Provide training to visiting medical, nursing and other healthcare students where appropriate. 18. Management of medicines at discharge from hospital To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes manage these changes without referral to a GP, perform a clinical medication review, produce a post discharge medicines care plan including dose titration and booking of follow up tests and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in care homes). Work in partnership with hospital colleagues (e.g. care of the elderly doctors and clinical pharmacists) to proactively manage patients at high risk of medicine related problems before they are discharged to ensure continuity of care. 19. Implementation of local and national guidelines and formulary recommendations Monitor practice prescribing against the local health economys RAG list for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs). Liaise directly with hospital colleagues where prescribing needs to be returned to specialists. Assist practices in setting and maintaining a practice formulary that is hosted on the practices computer system. Suggest and develop computer decision support tools to help remind prescribers about the agreed formulary choice and local recommendations. Auditing practices compliance against NICE technology assessment guidance. Provide newsletters on important prescribing messages to improve prescribers knowledge and work with the team to develop and implement other techniques known to influence implementation of evidence such as audit and feedback. 20. Medicines safety Horizon scan to identify national and local policy and guidance that affects patient safety through the use of medicines, including MHRA alerts, product withdrawals and emerging evidence form clinical trials. Manage the process of implementing changes to medicines and guidance for practitioners.

An exciting opportunity has arisen for a Lead Senior Occupational Therapist to join our team and provide leadership to our Stroke and Neurology OT service. The post holder will lead and deliver occupational therapy services alongside a dedicated team of OT staff, working on the hyper acute/acute/ rehabilitation stroke unit, and early supported discharge service. Early supported discharge operates as an outreach service from the stroke unit, enabling patients to continue therapy at home following discharge. The team is committed to delivering high standards of care working closely with the multidisciplinary team across the whole patient pathway.



We are seeking applicants with significant experience at a senior level, and who demonstrate specialist skills in Stroke/Neurology and rehabilitation. To provide clinical pharmacy services to wards and departments To be responsible for the clinical pharmacy service to specialist haematology ward patients Attend and contribute to the haematology ward round, and MDT meetings Contribute to the pharmaceutical care of patients admitted for acute oncology reasons. To be responsible for the safe management of Systemic Anti-Cancer Therapies (SACT) for patients who are admitted to the wards for any reason. To clinically verify prescriptions for SACT and supportive medication. To prescribe supportive medication where indicated. To give specialist advice and support to medical and nursing staff, and other members of the pharmacy team. To give education and training to nurses and other members of the pharmacy team To give advice to patients and carers To counsel patients and confirm consent prior to initiating SACT treatment To communicate effectively with primary care regarding the care of patients on SACT medicines. To act as a pharmaceutical and prescribing adviser, including the production and of review of medicines related guidelines and pathways To contribute to the safe introduction of new SACT To undertake prescribing within a defined speciality, and as part of wider medicines optimisation initiatives undertaken by the clinical pharmacy service. To undertake clinical pharmacy practice at an advanced level. To monitor compliance and advise on the implementation of National Guidance e.g. NICE, To provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required To contribute to the delivery and development of clinical pharmacy services across the Trust Actively participate within the multi-disciplinary and clinical pharmacy team To act as a source of expert advice on prescribing and medicines optimisation to medical and nursing staff within a defined clinical area To participate in clinical audit and other initiatives aimed at assuring high quality prescribing and pharmacy practice, within the context of Quality Governance

You will need to register if you do not already have an account Stock top up and medicine management audits Ensure medicine cupboards and clinic areas are kept tidy. Inpatient repeat top up as per standard operating procedure to designated wards. Check and audit out of hours crisis cupboards. Unpack stock medicines on receipt from pharmacy into ward and team stock cupboards ensuring standard operating procedures and trust medicine code is followed and accurate completion of all documentation for traceability. Work closely with Chesterfield Royal Hospital in line with our Service Level Agreement to support medication supplies to wards. You will answer queries on the supply of stock related medicines. Sort and organise return of medicines back to pharmacy as per department procedure. Participate in regular dispensing sessions at Kingsway Pharmacy which will include the dispensing of inpatient, leave, discharge and outpatient medicines including compliance aids and Controlled drugs, Carry out daily and monthly CD stock audits, issue of prescriptions and housekeeping tasks. You will also work within the stores, distribution and procurement team assembling and checking stock medicines, preparing medicines for distribution, stock take, ordering and receipt of medicines when required. Completed trust mandatory and in-house training Refer to job description for full role requirements You must hold a GPhC approved level 2 Pharmacy Support worker qualification and GCSEs or equivalent in English , Maths and dual award GCSE or equivalent in science. You must have significant experience of working in a pharmacy environment and be able to mentor and train junior healthcare professionals.
