The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. Greeting patients on arrival, booking them in and directing them to waiting area. Taking requests by telephone for appointments and home visits quickly and efficiently. Directing calls to the appropriate person and taking messages and dealing with them as set out in protocols. Advising patients of other services available (signpost). Liaising with GPs and Nurses and other staff within the Health Centre. Dealing with requests from clinicians as quickly and efficiently as possible, e.g. booking ambulances, telephoning patients, etc. Dealing with eConsult requests from patients, getting them triaged and liaising with patients as necessary. Issuing repeat prescriptions and dealing with requests for prescriptions as appropriate. Giving results of tests to patients following the confidentiality code. Dealing with incoming electronic mail and forwarding it to correct destination. Coding information into the Practice clinical system. Filing and retrieving paperwork. Updating patient records changes of names, addresses, telephone numbers, etc. Scanning & photocopying documents and ensure outgoing post is sent promptly. Handing out letters/ blood forms/ prescriptions/ sick notes, etc. Advising patients who wish to register at the Practice as to the correct procedure. Scanning paper documents and filing them in the computerised patients record. Handling requests for information from outside agencies. Entering the details of new registrations into the computer system from hand written forms and electronically received forms.

This is a great opportunity to join our team as the service delivers on the commitments of the NHS Long Term Plan (2019). You will be working within a multidisciplinary team that consists of CBT Therapists, Counsellors, PWPs, Administrators, Managers and Employment Advisors, and regularly engage with external stakeholders. To include; Health Checks o Blood Pressure, Height, Weight, BMI o NHS Health Checks including referrals Health and Well-being o Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being o Support patients to adopt health promotion strategies and encourage principles of self-care o Offer holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care o Weight Management & Advice Venepuncture Chronic Disease management o Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with chronic diseases CHD, Diabetes and Hypertension, Asthma, CKD Emergency Care o Prioritise health problems and intervene appropriately to assist the patient incomplex, urgent or emergency situations, including initiation of effective emergency care. Wound Management O Removal of stiches and clips o Meet the needs of patients presenting for wound care and use dressing formulary Mental health, adult and child safeguarding o Recognise patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health. Raise relevant information with the duty GP. o Recognise adult and child safeguarding situations and act appropriately Learning Disability annual check with clinician/GP Vaccination programmes o Provide vaccination and immunisation programmes for Adults following guidelines for flu, pneumovax, shingles, B12 Infection Control o Monitor fridge temperatures twice daily o Manage vaccination stock when required o Checks of emergency trolley, home visit bag and any other check as required Knowledge of IT O EMIS web o Docman o ICE o Any other package required with in the Practice Patient Profile Activities O Review patient data to work with the nursing team, to achieve focused patient healthcare o Active participation in achieving QOF and Enhanced Services targets and recall demands. • Salary: £37,500 - £43,000 + £2,000 after one year + ongoing bonus of up to 10%

The role

As a Functional Assessor, your main task will be to assess individuals and how their disabilities or conditions affect their work life. Drawing on your clinical expertise, you'll listen carefully and take down each person's medical history before performing a physical examination. You'll then produce concise, accurate reports for the Department of Work & Pensions (DWP) so they can decide each person's eligibility for benefits.

The role involves meeting people both face-to-face and via telephone/video. But whether you connect online or in person, you'll be sensitive, compassionate and able to ask the right questions while upholding best practice standards.

Rest assured that you'll have the time you need to make a precise assessment and give each person your whole attention. And while you'll work unsupervised, you'll be part of a wider multi-disciplinary team - support is all part of the package.

About you

• An GPhC Registered Pharmacist with a minimum of 12 months' post-registration experience within the past five years.
• Continuous Professional Development present on your CV.
• Great oral and written communication skills, with the ability to negotiate and respond to a rapidly changing environment.
• IT skills and experience of using a range of software
• Self-aware and focused on professional development
• Honest and able to maintain integrity and customer confidentiality
• Motivated to contribute to quality customer service, demonstrating empathy, respect and sensitivity in assessment delivery.
