The MHP will need to develop their skills and knowledge enabling them to draw on a range of therapeutic interventions, for example different psychotherapeutic models, non- verbal therapeutic approaches and other techniques adapted for use with children and young people. They will also need to develop an extensive knowledge of the full range of children and family services across all agencies. 7a Direct intervention by the MHP is undertaken according to the competencies of individuals. As a Band 6 clinician the expectation will be that the clinician takes a lead role in the delivery of the service and supervise the junior members of the team. Livewell Southwest is an independent social enterprise (Community Interest Company) providing integrated health and social care services for people in Plymouth, and community health services in South Hams and West Devon as well as some specialist services for those living in other areas of Devon and Cornwall. The post holder is responsible for the delivery of high-quality evidence-based care for acutely ill patients within their homes, with the aim of preventing hospital admission or facilitating rapid discharge. The post holder will liaise with the referring clinician when accepting patients for treatment and be supported by senior clinicians and clinical manager. Responsible for effectively assessing, planning, implementing, monitoring and evaluating patient care this will include admitting and discharging patients form the caseload. The Advanced Clinical Pharmacist for Virtual Ward is a new role within Livewell SW and the Pharmacy Team. The post holder with the support of the Lead Pharmacist for Virtual Wards will be leading and delivering a high quality, clinical pharmacy service to Virtual Ward areas, and working in partnership with our System partners to maximise seamless transfers of care and maximise best outcomes for the patients. The Business Development Manager role leads on the development of collaborations with industry, organisations and government partners within the BioPharma sector.
