The specific patient population to whom direct care is provided includes adults with a variety of different conditions. The incumbent must also be able to direct non professional staff's lifting, moving, and/or transferring of patients, and use equipment to provide patient care which usually involves lifting and/or pushing the equipment. The RN is responsible for the storage, proper cleaning techniques, and use of reusable medical equipment (RME). He/She is responsible for carrying out proper procedures for caring for patients in an isolation environment by following proper infection control procedures. Cheshire and Merseyside Adult Gender Identity Collaborative (CMAGIC) is a new NHS England pilot, establishing how Gender Incongruence can be best treated within a Primary Care setting. We are seeking a specialist nurse to support the delivery of this care and they will work closely with our MDT, local GP's, relevant partner agencies and of course directly with service users.

This is an exciting opportunity to be involved in developing not only a new service, but a new way of delivering care for a disadvantaged service user group. The role will likely involve travel, so a car user is essential.

To provide direct, clinical nursing care to the service users of the Cheshire and Merseyside Adult Gender Identity Collaborative (CMAGIC). This will involve direct and indirect care of services users of CMAGIC and an ability to work autonomously, as well as part of a wider team is essential. To be able to manage a caseload of patients and support patients throughout their journey.

What you will be doing: As a qualified Physical Health Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments. You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented. As a Physical Healthcare Nurse, you will be: Adhering to the NMC Code of Conduct and facilitate the compliance with local, Trust and national standards of practice. Developing and supporting physical health interventions used in the clinical area using knowledge of current best evidence-based practice. Engaging actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice. Working collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments. Monitoring and maintaining Physical Health Dashboards. Where you will be working: Location: Castleholme Lodge, 4 Bryan Road, Edgerton, Huddersfield, HD2 2AH. Castleholme Lodge is situated in Huddersfield with excellent transport links to the surrounding areas. It is undergoing a complete refurbishment to ensure best environment for service users and staff to thrive in. Castleholme Lodge will be a 20 bed Care Home with nursing, providing a range of mental health services for adults with severe and enduring mental health difficulties. It will form part of a cluster of Elysium services across the Yorkshire and Midlands region that work closely together in support of each other.
