MAIN DUTIES AND RESPONSIBILITIES The following are the core responsibilities of the Enhanced Nurse Practitioner. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. You will work within your own scope of clinical practice and: Work alongside the multi-disciplinary team (including GP support) in accordance with clinically based evidence, NICE guidance, the Direct Enhanced Services contract of the Network. To lead on the vaccination programme for SSWPCN in alignment with the practices and the clinical teams within the practices. Providing advice, support and guidance alongside leading the housebound and care home vaccination provision. Provide leadership and oversight to the community investigation hub (CIH). As required, be able to prescribe for the caseload of patients within your proven scope of practice. Using your knowledge, you will provide information and advice on prescribed or over the-counter medication on medication regimens, side-effects, and interactions. Be able to perform investigatory procedures, including laboratory and radiology. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. There may be a requirement to undertake home visiting and triage duties when required for the PCN (this would be in liaison with the practices within the PCN) Collaborate with other members of the PCN including doctors, nurses and AHPs, accepting referrals, using clinical reasoning skills to recognise when referral to them for specialist care may be required. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associate with patient health care and registration with the practice. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care. Enhance their own performance through continuous professional development, keep up to date with current evidence-based practice Impart own knowledge and skills to PCN colleagues to meet the needs of the service. Work in partnership with other clinical teams to collaborate on improving the quality of health care by responding to local and national policies and initiatives as appropriate. Participate in clinical audits, research projects, and implement changes as required, including the development, and updating of practice protocols / guidelines and procedures locally. Participate in the review of significant and near-miss events. Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in own Personal Development Plan (PDP).
