The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. Family members are taught to recognize and change behaviors and behavior patterns that impede blind and vision rehabilitation progress and how to cope with the psychological and social problems and their own individual stress.

The incumbent functions as a Psychologist in the Integrated Mental Health in Primary Care Program which is located at the Southeast Tucson Community Based Outpatient Clinic. The psychologist functions as a part of the Primary Care treatment team providing behavioral health screening and brief, time-limited behavioral health services related to depression, anxiety, substance abuse and other behavioral health issues for Veterans.

Major duties include, but are not limited to:

• Functions as an onsite and/or home tele-health practitioner, providing face-to-face and/or VA Video Connect (VVC) mental health psychotherapy services for the SAVAHCS Primary Care-Mental Health Integration (PCMHI) program for the Southeast Community-Based Outpatient Clinics (CBOC) , as well as virtual care as needed to other CBOCs and the Tucson Main Campus.

As a BHIP Psychiatrist you will provide psychiatric care (diagnosis, treatment, and crisis management) to Veterans with mental health and substance use disorders as part of an integrated team of psychiatric providers, psychologists, social workers, nurses, and clinical pharmacists throughout the SAVAHCS. The BHIP Psychiatrist is responsible for providing psychiatric care for a panel of approximately 500-525 outpatient Veterans with a wide range of mental health disorders using multiple modalities (in-person and virtual). Full-time BHIP Psychiatrists are expected to provide 30 hours of direct patient care per week. The BHIP Psychiatrist serves as a clinical leader and provides clinical consultation and assistance with patients assigned to the BHIP Team. The BHIP Psychiatrist provides cross-coverage for psychiatric colleagues in outpatient. The psychiatrist may participate in teaching and supervising Psychiatry Residents and Medical Students in the outpatient BHIP clinic and/or inpatient psychiatric unit. This position requires 3-4 weeks per year of home call (psychiatry resident is first call) and in-house rounding on the weekend. Weekend days are compensated by having weekdays off. The BHIP Psychiatrist must be able to obtain affiliate faculty appointment with the University of Arizona College of Medicine.

PACT Physician provides comprehensive care to a panel of adult patients, from young adult to elderly. This position will work out of the Women's Health Clinic at the Main Campus location in Tucson, AZ. Duties include but are not limited to the following:

• Works seamlessly with other health care team members to support VA quality and patient satisfaction initiatives.
• Provides direct patient care to optimize patient health including ordering diagnostic testing, prescribing medication including contraceptive medication, consulting with specialty services and utilizing computerized/electronic medical records.
• Provides patient care in accordance to professional standards, core values and commitment to service excellence.

Major duties include, but are not limited to the following:

• Conducts comprehensive nutrition assessments using the Nutritional Care Process (NCP), including nutrition focused physical examinations, to establish a nutrition diagnosis and determine risk and/or degree of malnutrition.
• Develops and implements Medical Nutrition Therapy (MNT) to address the nutrition diagnosis, evaluates and monitors the effectiveness and outcomes of MNT interventions. Modifies the plan as needed to achieve positive patient outcomes.
• Calculates nutrient requirements and necessary diet restrictions or food and drug interactions tailoring the diet prescription to each individual patient.
• Documents MNT in the medical record using the Assessment, Diagnosis, Intervention, Monitoring, Evaluation (ADIME) format.
• Functions independently and accurately when record keeping.
• Identifies patients in need of nutritional support and recommends the appropriate mode of support and communicates appropriately with the health care team.
• Participates in formal and informal interdisciplinary team meetings.
• Assesses educational needs and provides nutrition education/counseling for individuals and groups that is tailored to the Veteran(s)
• Utilizes virtual care technology to improve access to care.
• Maintains accurate record keeping of daily clinical activities and completes patient encounters using approved systems.
• Collaborates with the Food Operations Section and department leadership to enhance the Veterans' dining experience, support patient satisfaction, and serve as a liaison between clinical units and the Food Operations Section
