This position reports directly to the Assistant Chief of Physical Medicine and Rehabilitation (PM&R) at the VA Northern California Healthcare System. The Pain Section Chief, Physical Medicine and Rehabilitation Service (PM&R), is responsible for the administrative supervision, management, and direction of the PM&R Pain programs, including the Interventional Pain and Chronic Pain programs.

The principal duties and responsibilities include, but are not limited to:

Administration:

• Leadership responsibilities: Organizing Pain section staff meetings, communication both down to staff, across to other Section Chiefs, and up to PM&R Leadership, as well as crafting congressional responses, heads up and issue briefs, when requested to do so by VHA PM&R Leadership.
• HR Management: Recruitment; updates on Functional Statements and Position Descriptions; physician salary management and pay panel documentation; and Focused and Ongoing Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE) on privileged staff.
• Staff Management: Retention to include performance appraisal (mid-year and yearend) OPPE (twice a year on each provider); rewards and recognition; promotions; disciplinary actions and grievances; training requirements (including Talent Management System capability); employee health requirements and Protected and Administrative Peer Reviews.
• Time and Leave Management: Tour of Duty assignment and documentation, including VISTA/VATAS approvals of leave, timecard certification, and approvals of clinic cancellations.
• Business Planning: Systems redesign; manage service DSS mapping (workload); staffing needs; capacity and demand analysis resource needs (i.e., equipment, IT, supplies, space, etc.) followed by oversight of preparation of the annual Service Business Plan to be presented to Senior Leadership.
• Clinical Management (direct and indirect patient care): Open encounter and consult management, provider productivity, CPRS documentation requirements, oversight of clinic scheduling and access, ensuring staff productivity, oversight of relevant performance measures, and development of the new focus on the IDES Program and Department of Defense Collaboration, Utilization, Quality, and Patient Safety oversight through process improvement (system redesign).

The PM&R Pain is responsible for delivery of pain and rehabilitative care for outpatient PM&R Pain clinics including medication management and interventional management. This position is Pain Management, Opioid Safety and Prescription Drug Monitoring Program (PMOP) funded position.

The principal duties and responsibilities include, but are not limited to:

• Provides comprehensive pain and rehabilitation care to veterans through clinical evaluation and treatment within the outpatient Pain Program, Physical Medicine & Rehabilitation (PM&R) clinics and inpatient care as needed.
• Performs medication management including opioids, spine and other pain procedures, ultrasounds, non-medication management of pain including physical therapy, acupuncture, chiropractic, etc. and conducts Pain and PM&R clinics as needed and comply by all PMOP requirements.
• Supports the Opioid Safety Initiative (OSI) and opioid stewardship efforts in compliance with the Government Accountability Office, Office of Inspector General and Office of National Drug Control Policy.
• Ensures clinical appropriateness of the treatment of patients served, including timely and appropriate transition to other levels of care, notation of contraindications and precautions of care.
• Supervises and educates/trains PM&R residents in clinical activities relevant to PM&R and Pain Management.

I. Library Program Development, Implementation, and Administration
The incumbent is knowledgeable and skilled in all areas of library science and is delegated the primary professional responsibility to plan, develop, implement, evaluate, and revise the library program for this medical center. The incumbent:

• Develops and maintains knowledge-based systems and services relevant to the organization's needs.
• Acts as the point of expert knowledge in searching and evaluating the health care literature.

Functions as a resource to clinicians by searching medical databases to identify and retrieve knowledge-based information to resolve questions and controversies in diagnosis and treatment, clarify clinical decisions, decide upon the best course of care, and elucidate rare medical conditions. • Conduct electrocardiogram (EKG), phonocardiogram, echocardiogram, or other cardiovascular tests to record patients' cardiac activity, using specialized electronic test equipment, recording devices, or laboratory instruments including conducting exercise electrocardiogram tests to monitor cardiovascular activity under stress by using VA approved modalities of documentation.
• Design and explain cardiology treatment plans based on patient information, such as medical history, reports, and examination results.
• Provides daily Inpatient notes on cardiology patients and consultation notes for Emergency room patients on day of occurrence.
• Diagnose medical conditions of patients using records, reports, test results, or examination information.
• Explain procedures and discuss test results or prescribed treatments with patients for both inpatient and outpatient encounters.
• Monitor patients' conditions and progress and reevaluate treatments as necessary during hospitalization and upon discharge or transfer if indicated.
• Obtain and record patient information, including patient identification, medical history, and examination results.
• Order medical tests, such as cardiac stress tests, echocardiograms, electrocardiograms, Holter monitors, and consultations through VA or Care in Community for Cardiac Catheterization if warranted.

The Supervisory MSA plans and directs programs at Medical center primary care clinics and has full supervisory responsibility. Assignments include but are not limited to:

Assigning and evaluating the work of subordinate staff, resolving complex problems, evaluating new products, equipment , and systems to make recommendations for improved operations. Identifying educational or training needs, making final decisions on selections, evaluating performance and taking disciplinary action when necessary. Responsible for managing medical center primary care clinic grids and cancellations, letters and any other clinic scheduling needs in collaboration with the primary care administrative officer. Works collaboratively in the PACT care delivery model and performs all related duties; customer service and other duties assigned for the proper and timely treatment of patients and assure appointment schedules are maintained by MSA staff for one or more outpatient clinics. He/she must assist with clinic access contingency plans by adjusting appointment times, location, or dates as well as shift patients to other healthcare providers as conflict with staffing and/or coverage occurs. The supervisor makes assignments by defining objectives, establishing priorities and determining deadlines, assists with unusual situations which to do not have clear precedents. Plans and carries out successive steps of work assignments and independently handles scheduling problems and deviations based on person knowledge of clinic operations and policy, schedules, and provider's preferences.

1. Must have the ability to establish and maintain effective therapeutic relationships with Veterans engaged with mental health or substance use services and/or their families. The incumbent is able to work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems.
2. Completes thorough psychosocial assessments and diagnostic interviews to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable.
3. Responsible for contributing to the development of the patient centered treatment plan and setting achievable treatment goals with the Veteran, family members/significant others and other outpatient mental health clinical staff.
4. Must possess a working knowledge in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, and traumatic illnesses, common medications and their effects/side effects, and medical terminology.

1. Be responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility.
2. Enter all Veteran/family contacts in the electronic record using appropriate formats and templates and in accordance with Medical Center standards. This information will be entered in a complete, confidential, and professional manner to ensure information on the patient is shared with other VA staff. This information will be reviewed on a regular basis.
