Band 5 Community Registered General Nurse
Opportunity available for a Registered General Nurse to work within the community, Based in Worcester and Surrounding Areas.
The team sits within the community team with the role predominately providing and undertaking a wide range of physical health assessments and to support colleagues in undertaking physical health assessments. You will work collaboratively with primary health, develop and maintain links with GP practices and establish strong working relationships with other specialist services and signpost service users as required. As part of this role you to have accountability and responsibility for implementation of physical health pathways which offers physical health monitoring and intervention alongside medication administration. You will be providing service user groups to further assist in the process of promoting physical well-being of patients you will work as a specialist across the treatment and assessment interventions, overseeing the process of implementing physical health strategies and undertaking supportive roles in complex cases where physical health needs are identified. It is essential you have a Valid Driving License and access to a vehicle.
This role is to commence asap with the shift patterns being Monday to Friday
The Hourly Rate for this role is: £21 Per Hour
