To provide support to families within a variety of community settings under the direction of the Health Visitor, taking into account the diverse social, health and wellbeing needs of the community To plan and deliver health enhancing activities for children and their families to include working in groups, child health clinics, developmental review clinics, and on an individual basis To support breastfeeding and parent infant relationships applying the UNICEF UK Baby Friendly Initiative standards Maintain a family-centred public health approach as a framework for identifying and delivering health care interventions and key public health message To work in partnership with Children Centres, Early Years settings and other partnership agencies To promote the health and well-being of the local population To act as a supervisor/assessor for pre-registration nursing students and support new staff by providing suitable learning environments and experience Undertake child health needs assessments and health promotion activity Be responsible for managing own caseload and prioritise work to ensure all tasks are actioned in a timely fashion according to level of need Work as an autonomous practitioner taking responsibility for care given to babies, children and families but also work as a team member Take responsibility for monitoring own practice under local operational procedures, being responsible for own professional decisions and actions, including knowing when to refer to the senior colleagues when aspects of work are assessed to be outside of competence Be confident in the identification and safe management of vulnerable and safeguarding families, adhering to local safeguarding polices and escalating concerns when necessary Attend safeguarding meetings as requested by locality Health Visitor. Share health information, participate in formulating agreed actions for the baby/child and family and complete identified actions within set timescales as appropriate. Document all information and share with locality Health Visitor Manage all records in line with the Trust Health Records Policy, departmental Guidelines for Record Keeping and the NMC Code of Conduct Have a clear understanding of specific IT systems used within CNWL including Outlook and SystmOne. Ensure all training is attended to ensure proficiency when using the system Undertake risk assessments prior to undertaking home visits Implement plans of care using evidence-based criterion to rationalise professional decisions and actions. Monitor progress and adapt plans to maximise health benefits for client Signpost and refer to other specialist services for support and intervention Use a range of skills to communicate taking account of culture, ethnicity, language, disabilities, developmental age and environmental setting Be aware of childrens safeguarding policy and procedure. Ensure mandatory safeguarding training is up to date. Attend all teaching and supervision sessions to remain compliant with local and trust policy and guidance Actively participate in team discussions and help to support service development and projects Be responsible for revalidation and maintaining up to date and accurate registration with the Nursing and Midwifery Council and adhere to the NMC Code of Conduct Takes responsibility for own continuous professional development, maintaining professional knowledge and competence in line with personal development plan agreed at supervision and annual appraisal Ensure all mandatory training is updated annually or as specified, including attendance at service specific training as per local schedule Monitor own practice to ensure specialist knowledge is maintained through further education, study days, experiential learning and that practice is evidence base Recognise personal distress when highly emotive or unpredictable circumstances arise and seek support and advice from colleagues, management team and / or Trust provided services as preferred Consistently practice as a role model and contribute to creating and maintaining a high performing team
