We are looking to recruit a senior nurse with a genuine interest in physical health working with service users within the mental health service. The successful candidate will be self-motivated and able to work in a fast paced environment. Previous experience of working with service users prescribed Clozapine is desirable and an ability to act as a senior clinician within the Clozapine clinics.

The role will require the oversight and management of a very busy clinic, providing monitoring for the physical health of patients being prescribed Clozapine, there will also be times where you are required to carry out home visits so access to transport to complete duties of the role essential.

Extensive knowledge of running a clinic and managing clozapine medication is required, along side physical health monitoring such as ECG's, Venepuncuture, Pochi Machine operation and escalation of physical health concerns to the appropriate teams.

Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of a Registered Nurse or other registered care professional

Where appropriate work without direct supervision, at times delivering care independently in line with the individual’s defined plan of care, within designated parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed

Continue to develop and improve practical and theoretical knowledge, competence and skills whilst in clinical practice and maintain all evidence required

Contribute towards developing a culture of learning and innovation, developing high quality learning environments

Duties and responsibilities:

Clinical

Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual and their family.

Holistically support people to improve and maintain their mental, physical, behavioural health and wellbeing, promoting health and preventing ill health.

Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the ongoing assessment, planning, management and evaluation of care.

To recognise changes in a patients’ condition which require the intervention of others and understand when to refer to the supervising registered nurse and other senior colleagues for reassessment of patient care.

To work in collaboration with others to ensure the safe and timely discharge of patients with the aim of avoiding readmission.

To perform procedures as detailed within the NMC Standards for proficiency for Nurse Associate (2018) to meet the nursing needs of the patients in their care.

To ensure that high standards of nursing care are delivered and maintained.

To act and raise concerns when standards are not being maintained following local policy and procedures.

To act as an advocate for the patient and the family, including promoting and protecting their interests, privacy and rights, equality and inclusion.

To be involved in Trust Initiatives to improve patient safety and care.

Have demonstrated the proficiency to administer medications and undertake annual medicines management training, highlighting as appropriate any areas for personal development.

To be aware of and adhere to Trust Policies relating to the control and administration of medication.

To ensure that high standards of nursing care are given and maintained and to act when standards are not being maintained.

To work within the NMC code of professional practice, and within policies, procedures and guidelines of the Trust.

To be flexible to work within areas or departments to meet the demands of changing service needs.

Recognise and work within the limits of their competence highlighting as appropriate any areas for personal development.

Managerial

To communicate with the team leader/nurse in charge and collect data in order to participate in the smooth running and organisation of the department.

To support junior and senior colleagues in their roles and be resilient to manage change.

To support the team leader/nurse in charge in risk assessment and minimisation, ensuring incidents and near misses are reported appropriately.

To manage verbal complaints, inform team leader/nurse in charge and refer on when unable to resolve.

To participate in the investigation of incidents/complaints as required.

To establish and maintain effective communication, both verbal and written in every context/setting.

To recognise, prevent where possible, and manage areas of conflict, reporting to team leader /nurse in charge if unable to resolve.

To participate in staff meetings regularly.

To provide support and supervision to health care support workers to promote their learning and development within role.

To receive delegated duties from the supervising Registered Nurse and senior colleagues, accepting those that fall within their individual competency level, adhering to the NMC Delegation and Accountability document (NMC Code 2018).

The people we support love to get out into the community and enjoy a range of interests from Disney and bowling to shopping and walking. Helping to build and support positive relationships with others is important here. If you are good at cooking, managing a budget, or just passionate about helping others, then you could be part of our team supporting people to maintain or learn new skills needed to live their best life!

If you enjoy building meaningful relationships and think you could plan, communicate and deliver support with and alongside people, then this is the place for you.

You should be available to work a variety of shifts across the week including long days, sleep-in shifts and alternate weekends. You do not need to be able to drive for this role, however, a full UK Driving Licence is preferable.

Ability to commute/relocate: Sheffield S35 - You must be able to reliably commute for shift start times or plan to relocate before starting work

Gender is considered to be a genuine occupational requirement in accordance with paragraph 1 of Schedule 9 of the Equality Act 2010

Affinity Trust does not presently hold a Sponsorship Licence and while this status is reviewed regularly there is no present proposal to apply for such a Licence.
