A brand new and exciting opportunity has arisen for 2 x 1.0 perminant Band 4 Senior Support Workers to join our new Intensive Support Function Team for Children and Young People (CYP, ISF). The successful post holder will work with CYP and families who require intensive support to help support neurodiversity, learning disibilities, challenging behaviours and other health needs. We are looking for post holders who are passionate about neurodiversity and learning disibilities, keen to make a difference, highly motivated, enthusatic and have experience of working with children and young people who are complex in presentation.

The successful canddiates will be offered a breadth of opportunities to develop your skills and all additional training will be provided.

When joining the team, you will be provided with an induction into CYP services covering all areas of care as well as shadowing clinical staff during your induction period. You will have regular supervision and development opportunities to help support your skill set in the area of CYP LD and Neurodiversity.

This position requires you to work 9-5 Monday to Friday office based and some hybrid working. You will be based at our Manor Farm Site in Knowsley, where you will be able to park free of charge. BACKGROUND TO THE POST

This post will be based in Redditch / Bromsgrove.

The programme is specifically designed to engage with 16-25 year olds who are not in education, employment or training (NEETs) and provides them with an opportunity to re-engage in learning, participate in community projects, team work, attend residential visits and individual work placements.

The Team Leader will deliver the 12 week programme face-to-face with young people and work closely and effectively with local agencies and organisations to recruit team members. At the end of the 12 week programme, the post holder is expected to evaluate the programme and write reports.


DUTIES AND RESPONSIBILITIES

1. Recruitment of the team of young people, including liaising with training managers, employment services, careers officers, employers, youth organisations, hostels, Job Centre Plus, schools, colleges and other external agencies as necessary.
2. Deliver the team programme face-to-face.
3. Set team and students programme objectives, to meet both team and individual needs and deliver the expected learning outcomes.
4. Liaise with community organisations and agencies to plan, prepare and supervise appropriate team projects and individual placements for students.
5. Work with the Programme Co-ordinator to plan and deliver a quality assured team programme which includes all the required elements.
6. Provide leadership and guidance to the team, taking responsibility for their health, safety and welfare including appropriate disciplinary procedure, medical protocols and risk assessments.
7. Work with other Team Leaders and the Co-ordinator to plan and effect a strategic response to the needs of this cohort of students.
8. Conduct regular meetings for evaluation, review and assessment with the whole team on an individual basis.
9. Keep appropriate records of the team’s and each individual’s performance and progress by utilising the College’s internal quality systems e.g. registers.
10. Submit regular reports on the Team to the line manager and make significant input to staff team discussions, planning, review and evaluation and the curriculum area SAR.

12. Organise and lead 3 residential weeks with the Team during the academic year.
13. Other such duties as are necessary for the maintenance and development of the Team Programme.

We are recruiting for a Fixed term 12 month or secondment 12 months Band 7 Highly Specialist Speech and Language Therapist to work within children's community Speech and Language Therapy in Knowsley Service.

The successful applicant will provide high quality consultation, specialist assessment and intervention of speech, language and communication needs including provision of dysphagia.

They will support the development of the Speech and Language Therapy service and strive for continuous improvement.

To take a lead on and provide evidence-based Speech and Language Therapy assessment, treatment and intervention to service users who have complex needs.

An exciting opportunity has arisen in the Children's and Young Peoples Intensive Support Function Team for an experienced/skilled and passionate Clinical Team Manager.

We are looking to recruit a Clinical Team Manager to lead, manage and support the team in providing high quality care to children, young people and their families. This is an exciting opportunity to be involved in the service development of the intensive support function team, and the post holder will work closely in the Children, Young People and Families (CYP) leadership team as we introduce the service across Halton, Knowsley, St Helens and Warrington.

The intensive support function team will provide a designated resource to reduce/manage behaviours that challenge, working with the child/young person in their own residence, reducing/preventing the need for the use of restrictive practices, inpatient services, and out-of-area/residential placements. The intensive support function team works within a Positive Behavioural Support and Trauma-Informed Framework. This service will work across Halton, Knowsley, St Helens and Warrington.

Delivery of ISF input is throughfour core functions of support,in collaboration with the lead clinician/care coordinator from existing CYP service provision:

The post holder will carry managerial responsibility for Children, Young People and Families (CYP) Learning Disabilities/ Austism Spectrum Condition (LD/ ASC) Services within the Community Care Division and be responsible for the overall standards which will include referral management and access, assessment of need and delivery of care within CYP LD/ ASC Services.

The post holder will operate as part of an integrated multi-disciplinary team but be expected to be flexible in terms of travel to undertake duties.

The post-holder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care, and the setting of standards of care within that team.

The post holder will be aligned to CYP Learning Disabilities/ Autism Services and may be required to work flexibly across all divisional sites to provide response to the service needs of the physical and mental health clinical teams providing cover as directed by the Service Manager to ensure continuity of a quality service provision.

An exciting opportunity has arisen for an experienced Administrator to join the Intensive Support Function Team (ISF Team). This service will cover St Helens, Knowsley, Warrington and Halton Mid Mersey region.

The ISF Team is being implemented to help to improve the experience and outcomes for identified children and young people (CYP) on the Dynamic Support Databases (DSD) within the Mid Mersey region. The DSD is held by the Integrated Care Board (ICB) and is made up of CYP with a learning disability, autism or both who are at risk of an inpatient admission. This team willprovide a designated resource to reduce / manage ‘behaviours that challenge’ in the community to keep CYP in the community with their families, reduce crisis and reduce admissions to acute settings and Tier 4 beds.

The successful applicant will be strong team player with a high level of organisational and interpersonal skills. They will be well organised and have the ability to work to competing deadlines and priorities. The post holder will be required to work flexibly, have the ability to establish good working relationships and deal with a wide range of professionals via telephone, written and face to face contact.
• Daily retrieval of referrals received, opening, and uploading to Rio
• Attending weekly MDT meeting with clinical team to review referrals, recording of minutes and action plan
• Booking of appointments and diary management for Clinical team.

GENERAL

Provide an administrative, secretarial and clerical service to support the Team members, ensuring that letters and reports are accurately produced within agreed timescales, data is input to required standards, and an effective filing and retrieval system is maintained.

Effective liaison with multi-disciplinary staff/patients/carers/outside agencies ensuring accurate information is communicated as and when required, including reception/switchboard duties dealing with enquiries from clients and the public.

Responsibility for the processing of incoming and outgoing mail, including prioritising, sorting and action as necessary

Responsibility for arranging clinics, booking appointments and associated data entry including the retrieval and maintenance of patient records.

Responsibility for the maintenance of client database including the collection of statistics.

Arranging, when required, meetings and taking minutes, ensuring accurate transcription and distribution.

Ensuring the maintenance of the referral pathway process following Service procedures including liaison with other agencies.

Ensure appropriate records are maintained as directed by Line Manager.

Key responsibilities

• Raising sales invoices in line with customer invoicing terms to pre agreed systemised billing set up
• Raising free text invoices for one off re-chargeable costs
• Chase local authority accounts for payment of invoices and deal with queries
• Allocate cash received against ledger
• Manage queries with regions and customers. To work closely with Polaris region/home when invoicing customers and have proactive query resolution skills.
• Reduce current debtor days on ledger to agreed target, per customer, per region
• To be effectively and efficiently organised and maintain concise communication records.
• To ensure that we are invoicing for all services delivered on region / home managed.

Requirements

The ideal candidate must have the following skills and experience:

• Excellent interpersonal and communication skills with the ability to network at all levels
• Proven proficiency using Microsoft Word packages
