This is a new post with the aim of providing a broad introduction to the fields of cardiothoracic anaesthesia and intensive care medicine which can be tailored to suit the individual needs of the successful applicant. The successful applicant(s) will take part in the daily management of all ward patients, most notably through board rounds and ward rounds. Acute and immediate assessment/treatment of any unwell patients is expected, with escalation to senior staff. Standard junior doctor ward work, such as ordering/reviewing investigations and discharge paperwork, is part of the role. Good communication with patients, their advocates, and colleagues is an essential part of the job.The post involves taking part in the acute Cardiology on-call rota, supported by the on-call Cardiology consultant and resident Cardiology registrar. This commitment is 1 in 13 for evening ward cover, 3 in 13 for daytime weekend work and 1 in 15 for night cover. Training opportunities are readily available beyond the daily learning on the consultant and registrar wardrounds. There is protected weekly Cardiology teaching and a monthly CME (continuing medical education) session. The department provides opportunities to attend Cardiology outpatient clinics, gain competency in electrical cardioversion, attend echocardiography sessions and participate in the procedural aspects in the cardiac catheter labs (coronary angiography, PCI, pacemaker implants, complex device implants, ablation and TAVI). There are opportunities to undertake clinical audit and participate in research projects. The post can be tailored to the training needs of the successful candidate(s). There is an excellent track record of our Cardiology junior doctors securing national training numbers (NTN) in Cardiology, to become the Cardiologists of tomorrow. SPECIALTY AND DIRECTORATE INFORMATIONThe South West Cardiothoracic Centre opened in 1997. It is the tertiary referral centre for cardiothoracic services in Devon and Cornwall. The designated state-of-the-art building was opened in 2007 (The Terence Lewis Building), with integrated Cardiology, Cardiothoracics, Vascular Surgery, Critical Care and Anaesthetics. The Peninsula Heart Clinic, affiliated with the main hospital, was opened in 2019 and supports the work of the Cardiology directorate. Over 118 beds are dedicated solely to the Cardiology service, supporting activity of approximately 200 ablation procedures, 900 percutaneous coronary intervention cases and 1300 cardiac surgery operations. There are 17 Cardiology Consultants 7 interventional and 10 non-interventional. The 7 interventional Cardiologists provide the PCI service, including 24/7 primary PCI. The vast majority of diagnostic and interventional cardiac catheters are performed radially. Adjunctive technologies we employ include pressure wire assessment, intra-coronary imaging with IVUS and OCT, and rotational atherectomy. As the cardiothoracic centre for the region we take on complex and high risk PCI procedures, such as left main stem PCI, bifurcation PCI, graft PCI and CTO PCI (both antegrade and retrograde approaches). Intra-aortic balloon pumps are the mainstay of mechanical circulatory support when needed. TAVI is well established, with over 120 valves implanted annually.The 7 non-interventional Cardiologists offer the full spectrum of the lesser-invasive Cardiology tests/treatments. The echocardiography department is nationally accredited with the British society of Echocardiography. The service is well developed and offers dobutamine and bike-exercise stress echo, transoesophageal echo, bubble echo, ACHD echo and valve clinics. Over 100 complex devices are implanted annually, including S-ICD, and the device extraction service is growing. RELATIONSHIPSThe post holder will be professionally accountable to the Medical Director and managerially accountable to the Service Line Director.

Utilising specialist knowledge of complex NHS/Trust accounting systems, processes, regulations, financial techniques and relevant legislation; NHS Tariff guidance and Contractual requirements, together with general commercial awareness, for individual Services to: o Provide strategic and business planning advice to Service Line and Care Group Management teams; o Encourage and influence Service Line Management Teams to ensure cost-effective decision making by management, using high developed persuasion, motivation and negotiation skills; o Investigate highly complex financial queries on income and expenditure, carrying out regular complicated analytical reviews of the Service Line financial position, predicting income, developing and using complex models and multiple inputs including activity projections and case mix issues; o Interpret highly complex financial data and give advice on financial risks to ensure the budgets are achievable and owned by the Service Line Managers and Budget Holders. o Assist and advise Service Line Management Teams in the execution of plans to achieve the performance targets set out in the Trusts Annual Plan, within the financial resources allocated to the Service Line for that purpose o Oversee the accuracy and integrity of Service Line financial information relating to service line reporting and reference costs in particular; o Identify business opportunities and threats arising out of market analyses and work with service lines to identify financial impacts and mitigating actions o Thoroughly review monthly contract monitoring reports with service lines to ensure robust capture and coding of income. Work with service lines and Accountants to develop robust activity and income forecasts based on changes to demand, RTT requirements and capacity constraints. o Benchmark the Service Lines against other similar hospitals using nationally recognised activity and management information and peer data (such as GIRFT and the Model Hospital) - identify areas for improvement and advise Service Line Management Teams and work closely with Service Line Management teams to effect such changes.
