Outpatient Pathway

· Administer all new referrals received via the E referral service, ensuring all are electronically triaged by the appropriate clinical staff member and action asrequired.

· Process and log all non-Choose and Book referrals on EPR, including registering new patients in addition to updating existing patient details.

· Book new and follow-up outpatient and diagnostic appointments, ensuring that capacity is proactively and efficiently used. This includes booking patients into the correct clinic to ensure that they are seen by the most appropriateclinician.

· Reschedule outpatient appointments as a result of patients calling the service, in line with the 18-week RTT / cancer / screening targets and agreed local processes inrespectto patient cancellations.

· Actively review patient DNAs,liaisingwith the appropriate clinical staff and reschedule patientappointmentsinlinewithagreedlocalprocesses.

· As required, completecliniccancellation forms and ensure appropriately authorised(minimum6 weeks'noticerequired) before processing thecliniccancellation on EPR andrescheduling of patient appointments.

· Addingadditionalcliniccapacity on EPR as directed by management, toincludethe useof adding appointment slots on EPR that can be booked via Choose andBook.

· To be responsible for actively managing the OP PTL and Incomplete lists.
General
· Demonstrate high levels of customer care and be an ambassador for customer care within the Trust.

· Provide a robust administration function that underpins the delivery of a high quality service and maintain effective working relationships with clinical, nursing and administrative staff.

· Answer all telephone calls in a timely manner and action as appropriate.

· Maintain patient confidentiality at all times.

· Be flexible in your approach to work, such as covering other job roles at an appropriate grade or site, and to include varying working hours to ensure the service maintains a minimum level of cover during its core hours of 08:00 to 17:00.

· Retrieve notes and ensure all patient information is available in a timely manner for all appropriate outpatient appointments, diagnostic appointments and inpatient / daycase procedures, checking demographics at every stage and locating lost notes where appropriate.

· Support the Team Leader, Patient Pathway Manager and Service Manager in the investigation and resolution of patient queries and complaints received direct to the service or via PALS / Complaints Department.

· Book interpreters for patients prior to appointments and admissions as and when required.

· Liaise with Clinical coders to ensure patient notes are available so that all patient episodes are coded accurately and promptly.

· To support and contribute to on-going and future service development projects supporting continuous improvement of the services we provide.

· Any other duties at the request of the Team Leader / Patient Pathway Manager / Service Manager, which may be needed to fulfil the objectives of the post, which are appropriate to the grade.

Your core role in Clinical Trials Pharmacy would be to:

Obtain and maintain Good Clinical Practice (GCP) training, apply principles of GCP in the management of clinical trials and work within the Medicines for Human Use (Clinical Trial) Regulations 2004 and EU directive relating to Clinical Trials, communicate with Investigators, Research Nurses, Trust R&D, Sponsors, Clinical Research Associates and Pharmacy staff during the set-up and running of a clinical trial, ensure clinical trial protocols and SOPs are followed during dispensing of clinical trial investigational medicinal products and other drugs included in the protocol, order and receive in clinical trial drugs, process receipt paperwork and acknowledge receipt on IVRS (Electronic Medication Supply) systems, check stock levels and expiry dates of clinical trial drugs and keep balance logs up to date, assist with the packing down and re-labelling of clinical trial drugs if required, assist with the preparation for monitoring meetings, audits and inspections, maintain Clinical Trial files ensuring items can easily be located and are presentable, process returned clinical trial drugs and ensure the paperwork is kept in order, participate in the statutory temperature monitoring of clinical trial stocks, assist with the destruction of clinical trial medication, assist with the closedown and archiving of clinical trial files, represent the Clinical Trials Team to update the dispensary staff at meetings, liaise with Research Nurses to update them on trial progress, stock levels etc, assist in the collection of data as requested by the Senior Clinical Trial Technician or Clinical Trial Pharmacists, assist in maintaining financial records, including the generation of invoices, checking payments etc, assist with general administration such as organising set up meetings etc and be a point of contact within the Clinical Trials Pharmacy Team, maintain clinical trial filing systems and drug accountability documentation for clinical trials, use the computer system and other information technology in all areas of Pharmacy.

You will be expected to work as part of a team in order to assist the doctors with diagnostic procedures and facilitate the efficient running of clinics. You will required to assist in the running of specialised clinics such a our 2 week wait hysteroscopy, vulval and pelvic mass clinics, our colposcopy clinics supporting clinicians carry out treatments following abnormal smears and our general gynaecology clinics that include urogynaecology and endometriosis clinics. This role covers a wide variety of services within women’s health. You will be part of the nursing team to support the department sister in the running of the unit. You will assist in the maintenance of the patient environment in terms of cleanliness, tidiness, auditing, and the restocking and ordering of supplies. You will be part of the team also supporting our Pre op assessment nurses to carry out assessments on our patients that fit certain criteria following out patient hysteroscopy being booked for general anaesthetic.

Staff Management:
• To be responsible for the line management of the OCDEM Secretariat and admin team, providing first level EPR support, ensuring that adequate cover is provided in all areas of activities.

• To act as the first point of contact for written and telephone enquiries on OCDEM matters for patients, their relatives, the call centre, outpatient’s, medical secretaries, consultants, and junior medical staff. To deal with telephone contact with members of the public appropriately and sensitively in situations which may be stressful for both the patient and the staff member involved.
• Development and implementation of new or revised administration policy and procedures.
• Taking and relaying as appropriate, telephone enquiries and messages, using initiative and prioritising skills when disseminating information.
• To assist the Clinical Unit Manager and Patient Access Manager in the writing of reports and documents.

• To carry out surveys/audits as requested.
• To ensure that patient case notes are correctly maintained by all staff and that high quality standards are maintained at all times and tracked appropriately using the relevant software.
• To manage the information flows between the OCDEM Secretariat and the Waiting List office.
• To receive and make telephone calls and take appropriate action/discuss with appropriate staff maintaining a friendly, professional and courteous manner at all times. These conversations can often be of a distressing nature.
• Maintaining physical and electronic filing systems as well as other general ad hoc office management procedures.
• Must exercise judgement to analyse and resolve problems.
• The post holder is required to work unsupervised and is expected to plan and organise his/her own time.
• Provision of information and advice to patients and relatives about admissions and appointments exercising judgement when dealing with patient enquiries; analyse and resolve patient problems.
• Be responsible for communicating cancellations to all relevant parties with the reasons for cancellation including patients and relatives.

In summary, the post holder will provide expert nursing care to patients enrolled into clinical trials at the Oxford BMT Research service.

Staff Management:

Occupational Health Adviser will be able to:



· Support the protection and promotion of the health and wellbeing of current and prospective employees, and those from contracted and other organisations as per Trust arrangements.

· In accordance with Trust / Department policy and procedures, to deliver OH clinical activity such as, Pre-placement health clearance activity, and risk management & exposure related advice and intervention. The post -holder may be asked to support the service with management referral consultation and reporting.

· As an Occupational Health Adviser, provide evidence-based opinion, intervention and guidance to Trust management and partner organisations to support their line and risk management responsibilities, assist in safeguarding against risks arising from work and the working environment and to influence the application of safe and healthy work practices.

· Take responsibility for specific areas of occupational health service delivery and actively promote the work of the OH service by acting as a key link for managers and HR to named clinical programme groups or external clients.

An exciting opportunity has arisen for a Ward Clerk to join the Specialist Surgery team. We cater for a variety of specialities including Plastic reconstructive surgery, ENT, Ophthalmology and Maxillofacial surgery. Our friendly department consists of a 30 bed mixed ward and a day surgery unit for elective and emergency surgery and GPRU and HAPI clinics.

The post holder will need to have administrator and have good communication skills. You will need to be organised and have an ability to manage time effectively. You will need to have experience with patient management systems and be prepared to undertake further training as required.



The role will involve meeting and greeting patients and their families arriving on both reception areas, inputting patient details onto the patient administrative system, requesting and tracking patient notes, scanning notes, taking telephone calls, admitting and discharging patients on the patient management systems.

The successful post holder will play a vital role in ensuring clerical and administrative support if maintained and upheld in Specialist Surgery in-Patient. The post holder expected to have good interpersonal skills and communicate with the multi-disciplinary team. You will be working very closely with the nursing team in general and specifically, provide administrative support to the senior nurse team.

Previous experience would be advantageous, but full training on the hospital systems will be given.

Service Manager – Adult Speech and Language Therapy

The post holder will be a specialist Speech and Language Therapist with an interest in voice and upper airway disorders. The post holder will work closely with the Speech and Language Therapy Clinical Lead for Voice/ENT to assess, diagnose and manage voice and upper airway disorders for outpatients. They will be an integral member of the multi-disciplinary team, providing delivering a service which demonstrates evidenced based, effective, and efficient clinical care for this caseload. The postholder will work within the SLT team, the Special Airways MDT, ENT team, to deliver this service in the outpatient setting.

The post holder will:

· Use highly specialist clinical skills to identify, assess, diagnose and manage a specialist caseload of adults with voice and upper airway disorders including chronic refractory cough and Inducible Laryngeal Obstruction.

· Be part of the Speech and Language Therapy Department, and work closely with MDT colleagues in aspects of patient care

· Contribute to audit, quality improvement and service development projects in these clinical areas.

· Independently support students on placement for clinical teaching and supervision

· Provide necessary cover for the caseloads of other Speech and Language Therapists within the Trust during periods of annual / sick leave.

Main Tasks and Responsibilities

Patient Care
1. To assess, develop and implement specialist evidence based Speech and Language Therapy interventions. To make appropriate clinical decisions following assessments, including differential diagnosis on the basis of evidence from assessment, and to develop care plans based on best practice.
2. To use clinical skills to generate written reports reflecting specialist knowledge.
3. To provide appropriate specialist intervention and evaluate outcomes. The post holder will use these specialist skills to generate appropriate strategies for caseload management for complex clinical issues.
4. To provide advice to others regarding the management and care of patients with voice, communication and swallowing disorders.
5. To work collaboratively within a multi-disciplinary team to achieve patient centred care.
6. To liaise with a wide range of professionals across a variety of locations and agencies.
7. To maintain intense concentration in all aspects of patient management for prolonged periods. In particular, to monitor auditory, visual and kinaesthetic aspects of patients’ communication, adapting and facilitating according to perceived needs.

Policy / Service Development / Implementation
1. To represent Speech and Language Therapy at multi-disciplinary team meetings, ensuring delivery of coordinated care.
2. To be aware of, adhere to and implement Service and team plans and policies including:

· Providing regular, timely statistical data in keeping with Trust requirements

· Maintaining high quality clinical records in accordance with Service and Trust Guidelines and within data protection guidelines

· Ensuring understanding and adherence to Trust health and safety requirements, including attendance on mandatory training courses

· To work within infection control and health and safety guidelines in order to deal appropriately with unpleasant conditions related to client contact as they arise e.g. exposure to body fluids, infectious conditions encountered on a regular basis

· Participation in the Trust incident reporting and risk management procedures

3. To use specialist knowledge to inform Service / policy developments as appropriate and to implement policy within own specialist area.

Financial and Physical Resources

Would you like to work in a forward thinking, large cancer pharmacy team which includes Consultant Pharmacists in Cancer as well as Palliative Care, Highly Advanced and Advanced Cancer pharmacist, as well as the first Advanced Cancer Pharmacy Technicians in the country? This exciting post promises to bring you an opportunity to work with national and internationally recognised expert pharmacist in the cancer and palliative care field. We provide local, regional and national cancer services including CAR-T, BMT treatments, gene therapy and we are also 1, of only 5, bone and soft tissue specialist centres in England.

Our team have a range of oncology and haematology pharmacy non-medical prescribers who work alongside consultants and other members of the MDT to review and prescribe SACT.

OUH is a clinical trials centre and we are involved in early and late phase research. As a teaching hospital, we believe that research should be embedded within pharmacist roles and will support with research opportunities, including PhD.

We welcome applications from band 6 pharmacists who will be completing their clinical pharmacy diploma’s/MSc’s shortly.

Key roles include:
• Delivery of high quality pharmaceutical care to adult cancer in-patients, day-case, and out-patients.
• Participation in pharmacy services to the Day Treatment Unit to optimise patients’ medication and reduce medicines wastage.
• Clinical checking of systemic anticancer treatment (SACT) and SACT clinical trial prescriptions.
• Participating in multidisciplinary ward rounds
• Pharmacist led oral anti-cancer treatment education clinics

• • To provide a ward-based clinical pharmacy service according to Trust agreed standards
• To provide clinical pharmacy cover to other ward areas within the team on a reciprocal basis
• To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation
• To monitor medicines use within the ward area. This includes recording of significant clinical interventions and risk management.

• To contribute to the development of the Trust cancer information systems in accordance with agreed objectives
• To be involved in the preparation of new drug submissions for the Trust Medicines Management and Therapeutics Committee with Consultant colleagues
• To assist with review of sections of the Trust formulary relevant to the specialist area
