You will work across the new paediatric integrated unit, which opened in May 2022. We have created a purpose built, state of the art unit for children's services. Our patients and staff alike have designed it. This unit includes a Critical Care Unit, The Inpatient Ward, Short Stay Unit, Day Surgery Unit, Ambulatory and Oncology. We provide services for a diverse population in Croydon working closely with teams specialising in Asthma, Diabetes, Sickle cell, Epilepsy, Oncology and Neurodisability. Senior Acute Occupational therapist

We are looking for a keen OT to join our Acute Therapy Team. The team consists of Occupational Therapy, Speech and Language Therapy and Physiotherapy staff. We provide inpatient therapy to support patients recovering from acute illness or surgery, supporting discharge planning. About the role:

This is an exciting opportunity for an enthusiastic and dynamic Care Coordinator to join the Croydon Early Intervention Service (COAST). The post is open to nurses, OT's and social workers.

The post-holder will conduct assessments of newly referred adults to the Early Intervention Service who are experiencing First Episode Psychosis (FEP). They will undertake the responsibilities of a care coordinator within the Care Programme Approach for a defined caseload of service users and their carers. They will provide evidence-based psychosocial interventions and will focus on engagement, recovery and social inclusion as well as addressing physical health care needs.

The successful candidates will be experienced registered professionals who are able to demonstrate a specific interest in working with people with developing psychosis.

1.Assessment: To be responsible for the assessment of newly referred adults with First Episode Psychosis and the subsequent presentation of findings to the wider team. To work with diagnostic uncertainty, providing ongoing comprehensive assessments and outcome monitoring for all service users within caseload. To undertake risk assessments regularly, involving all other health professionals/agencies involved. To ensure that risk assessments are regularly updated, are meaningful and are reflected in care plans.

2)Engagement: Assertively outreach difficult to engage clients. To meet service users regularly, as far as possible at a venue of their choice, be this at home, CMHT centre, education, work or recreational establishments to assist with engagement and encourage participation in treatment
delivery as well as facilitating independence in all aspects of daily living.

3)Support:To provide ongoing care coordination of a designated caseload of service users who will be experiencing their first episode of psychosis. To provide support and advice in relation to service users regarding the issues concerning them. To work collaboratively with service users focussing on a recovery approach. Provide information to help clients make informed choices.

COAST is a multi-disciplinary team providing a comprehensive service to people who may be experiencing psychosis for the first time, living in Croydon. 7)Communication:To maintain accurate clinical documentation on designated caseload ensuring case notes are up to date as per guidance and in line with electronic patient journey system. To ensure effective verbal and written communication with all involved with delivery of care. This is an exciting opportunity for qualified CWPs to develop as a Senior Wellbeing Practitioner and supervisor, who will have been qualified for at least two years by January 2024.

6. To work in accordance with the Council & Trust’s policies to eliminate unlawful discrimination and promote equality and diversity in the workplace. 7. To adhere to the Infection Control Policy, procedures and guidelines.

8. All clinical staff hold responsibility for ensuring they have sound knowledge of standard infection control precautions and that no omission on their part or within the sphere of their responsibility is detrimental to the interests or safety of their patients, visitors and colleagues.

9. To comply with the Council & Trust’s Safeguarding Children and Vulnerable Adults policies, procedures and protocols. All individual members of staff (paid or unpaid) have a duty to safeguard and promote the welfare of children, young people and vulnerable adults.

10. To work within the relevant Professional Bodies Code of Professional Conduct and Scope of Professional Practice.

11. To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibilities of the post.
