The post holder will work as Administrator for the Hospital Discharge Team based at RBFH, youwill be an integral member of the multi professional team and will be working with patients based at the RBFT in supporting the discharge process to a community setting.
The postholder will need to work closely with key stakeholders within the RBFT Trust – delivering a community driven, discharge to assess approach to discharge – enabling the effective flow of patients into the right service at the right time.
The team will work to ensure a seamless interface between RBH colleaguesidentifyingpatients medically fit for discharge, and then ready to leave hospital. As Discharge Administrator you will ensure successful andtimelyonward transition to the next step in a patient’s pathway – this will include liaising with ward-based Discharge Co-ordinators and Occupational Therapists RBFT ward staff, social workers, and our systems partners mostly Berkshire wide and other outreach areas.
The HDT administrator will work with and support the Hospital Discharge Team to manage patient discharges - takingappropriate actionsto achieve a safe andtimelydischarge.
This will include the coordination of care for patients referred into the HDT service and to aid clear communications to multi teams, local authorities, and outside agencies to ensure the facilitation of safe discharges to a community setting taking direction for the senior HDT team
The post holder will work as a Team based at Royal Berkshire Hospital
• Be the point of liaison for service users receiving care coordination and interface with all relevant professionals asappropriateand escalate to senior team asappropriate.
• Demonstrate self-directed learning, actively seeking role development opportunities to enhance practice, knowledge, and role progression.
• To deal in a professional, helpful, and sensitive manner with service users and colleagues, staff and other agencies by telephone or face to face, taking messages, in communicating with service,regardingreferring and other issues asappropriate, and answering routine enquiries
• Maintaining a high standard of record keeping.
• To manage and prioritise workload daily and deal with the competing demands of the HDT team staff.
• Demonstrate an awareness of the limits of own practice and knowledge and when to seekappropriate support/advice.
• Be actively involved and support with QI projects within the service by undertaking associated administrative tasks.
• Responsible for the ordering of supplies as directed by HDT staff.
• Organise all outgoing mail; ensuring mail is ready for collection as given times. Sort and distribute incoming mail.
• Maintain effective communication across the service and deal with any enquiries, both internal and from stakeholders outside of the organisation, making referrals as appropriate
