South Kent Coastal Mental Health Together+ Team are currently looking to recruit 2x Band 3 full-time Support Time Recovery workers; the posts will be based at Coleman House in Dover.

Our service focuses on promoting recovery for those people diagnosed with severe and enduring mental ill health, as well as providing services to Older Age clients with ongoing mental health needs and dementia.

We provide comprehensive multi-disciplinary services and work closely with local specialist mental health teams (e.g. Early Intervention Service, Home Treatment Teams, MIMHS, Dementia Crisis team) tertiary service providers, primary care, Kent Police, local statutory and non-statutory agencies, service users & carers.



The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

The main elements of community mental health support work focus on collaborative working with service users to promote engagement and to facilitate an optimum level of independence and self-management. Our expectation is that you will endeavour to establish therapeutic relationships within which to address recovery goals in order to help service users to gain access to a range of resources and services to facilitate involvement and connection with the local community. Work closely with occupational therapists and to assist nursing staff with the effective functioning of a depot/clozapine clinic that will include well-being checks. Visiting people at their own homes as required.

People experiencing a mental health crisis will often attend an Accident and Emergency department in the absence of knowing where else to go to receive the care, support and treatment that they need. This has been recognised across the system and there is significant investment in Kent and Medway to provide people with an alternative pathway to attending A&E, such as Safe Havens.

The triage nurse will provide triage, advice and co-ordination of support services through liaison with relevant health, Voluntary Community Social Enterprise (VCSE) and social care services, for any patient with a mental health only crisis presentation at the emergency department. The role has been developed to meet the patients needs in a timely and effective manner, at point of contact and to maximise patient experience and minimise preventable waits in A&E. This is to ensure that their crisis support is delivered in an appropriate way.

You will be able to work as an autonomous practitioner in liaison with multi-agency staff to improve integration, update working practices and models of care, and promote recovery approaches, least restrictive practices and coping skills in crisis. The triage nurse will provide mental state and risk assessment of people who present at A individuals with complex needs in identifying immediate needs in times of crisis.
• The post holder will promote choice and hopefulness and ensure a recovery orientated approach.
• The post holder will promote and initiate any actions to safeguard vulnerable adults.
• The post holder will be responsible for the initial assessment of care needs of patients presenting with a wide variety of clinical conditions without direct supervision. Following this, the post-holder will be responsible for planning and referring on for appropriate follow up where necessary.
• The post holder will be expected to participate fully in quality assurance initiatives, with particular emphasis on a patient first approach when required.
• The post holder will act as a role model in providing a service for people with mental health problems for all staff within the A&E/main hospital.
• The post holder will participate in audit/research projects associated with the work of the service when required.
• The post holder represents Mental Health Trust in the general hospital setting and will play a vital role in communicating

Kent and Medway NHS and Social Care Partnership Trust has been working with acute hospital partners to develop this pathway, in order to prevent the need for people in a mental health crisis needing to attend the local A&E department. An important aspect of this pathway will be the ability to triage and assess people at the front door of the A&E department and divert them to alternatives that better suit their need, such as the Safe Havens.

We are therefore recruiting to the Hospital Liaison teams across all acute trusts in Kent and Medway in order to provide this vital front door service.
• The postholder will act as the first point of contact for all people attending an A&E department with a primary mental health need.
• To identify the most appropriate pathway for all patients first attending the A&E department based upon clinical need.
• To facilitate access to others service and sources of appropriate support as appropriate, such as Safe Havens
• To act as a resource for advice and expertise to staff, patients, carers and other agencies including primary care and VCSE colleagues
• To work in conjunction with the Team Manager and Service Manager to ensure that targets are achieved.
• Ensure that a “Duty of Candour” is maintained with services users/carers at all time.
• Ensure the service is delivered safely and in accordance with the Trust policy and legislation.
