Role:Field Care Supervisor

As a Field Care Supervisor you will be responsible for the ongoing field supervision of domiciliary and continuing healthcare support workers and will ensure that patient or service user requirements are met whilst maintaining compliance of care and nursing standards in accordance with AHG and Local Authority policies and procedures.

You will be required to provide day-to-day support to the Operational Network, assisting with the assessment, set up and review of health and social care packages, and you will be required to ensure that a skilled workforce is in place and they are able to deliver safe quality care to all clients.

As the Field Care Supervisor your responsibilities will include:

Ensuring that the client receives the appropriate care package and that continuity of care is a priority.

Providing telephone support to clients and care staff and where necessary, to work as a “hands on” carer in setting up the packages in the initial few weeks.

To assist the Branch Manager in auditing the standard of service delivery to clients.

Assist the Branch with Care Workers interviews & compliance

Assist with Risk Management training to all existing Care Workers

Complete the Risk Assessments and care plans of all domiciliary packages as well as the Non Case Managed & Level 1 Continuing Healthcare packages

Qualifications & Experience

Ideally possess a minimum of NVQ Level 2 in Health and Social Care

You must demonstrate commitment to working within a team and have a flexible approach to working hours and practices.

This role will involve an element of on-call duties.

Driver with access to a vehicle.

Shifts: 3 days per week, 7am-7pm or 7pm-7am
Rate: £24 umbrella an hour (£19 PAYE an hour)

About the Role

As a Telecare Officer, you will be responsible for providing crucial support and assistance to individuals in need, particularly during day and night shifts on a rotational basis. Your primary duties will involve handling calls, responding to emergencies, and ensuring the safety and well-being of individuals within the community.

Key Responsibilities

• Work day and night shifts within a structured 7am - 7pm or 7pm to 7am shift pattern
• Respond promptly to calls and emergencies
• Assist vulnerable adults in an Adult Social Care (ASC) setting
• Maintain a high level of professionalism and empathy in all interactions

Qualifications & Experience

• Previous experience in a similar role preferred
• Experience working with vulnerable adults in an ASC setting is advantageous
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Excellent problem-solving skills
• Hold a clean driving license

Finance Responsible for achieving the best impact from well-managed finances by: o ensuring the accuracy of any submissions required by the ICB/other authorities to claim all funding entitlement and to ensure that all monies due are received promptly o reviewing and approving operational services spend, monitoring income and expenditure and identifying and reporting to the CD and Network Board any significant deviations from forecast o planning and monitoring income and cash flow to ensure that income is maximised and that funds are available as required Present financial forecasts and budget reports to the PCN Board as required Ensure financial risks are recognised and discussed with the Clinical Director and Network Board members and appropriate action taken Ensure recommendations to the Board regarding allocation of resources are fair and equitable Manage the PCN bank account (currently a sub account within a Practice account) and all delegated budgets with propriety in line with all relevant organisational procedures Liaise with accountant and the Clinical Director to produce year-end accounts and present these to the Board in line with Network Agreement. Relationship Management Ensure engagement of member practices in accordance with the Network Agreement to: o maintain support for agreed PCN-wide activities o foster a culture of collaboration to improve patient outcomes, and resilience and capability across member practices o embed the PCNs ethos and values Develop, agree and deliver an internal and external communication strategy Represent the network at meetings in the community and to the media as required. • Oversee and manage the robust financial operations of children's social care, ensuring compliant spend and accurate financial forecasting.
• Deliver digital transformation, leveraging data to drive strategic commissioning and innovation.
• Enhance reporting capabilities, ensuring insights are actionable and influential at all levels.

You will also support with projects alongside external service providers and internal teams to develop key areas of our business.

You will bring:

• Proven leadership experiencein data and analytics, ideally within a matrix management structure.
• A strong track record of building and developing teams, creating clear pathways for success.
• Expertise in designing and implementing strategiesthat maximise the impact of data.
• Experience managing digital transformation programmesthat improve processes and outcomes.
• Excellent communication skills, with the ability to influence stakeholders and foster collaboration
• Extensive experience in finance and/or procurement operations, including both internal/external spend, forecasting and demand management.
• Organisational and administrative skills to support and manage a customer focused business support team.
