The Associate Chief of Staff, Geriatrics & Extended Care will be responsible for consults, admissions, discharges, and routine care of the Veterans in these programs. He/she will also help coordinate care with other members of the Interdisciplinary Team and with Specialists inside and outside the VA and provide education on disease process, medications, and care needs to Veterans, families, and staff. Will participate in screening Veterans for CLC admission, both hospice and other treating specialties within CLC.
