Main Purpose: To provide skilled high quality care to children and young people with complex medical needs, in the home or community setting. To carry out delegated clinical tasks providing support for children with complexhealth needs and those dependent on technology, following individualised care plans developed by the nursing team, having become competent through an intensive and on-going training program and with support and guidance from registered nurses. Main responsibilities: 1.Work as a member of the Childrens Continuing Care Team (CCCT) providing care shifts to support families of children with complex care needs, carrying out delegated clinical tasks with supervision from CCCT Nurses.2. Provide day or night care shifts as per a rota determined by each childs allocated care package, in the childs home or community e.g. school, place of respite.3. Provide a high standard of care at all times, meeting the individual needs of the child or young person and being attentive and alert throughout the shift to ensure the childs needs are fully met.4. Work closely with families and CCCT to promote the childs well-being.5. Accurately observe and record the childs condition, noting any changes and initiating appropriate action as required.6. Maintain up to date and accurate records in accordance with NHS standards for records and record keeping.7. Report any changes in the child/young persons condition or potential hazards in the environment to the family and the On Call/Lead CCCT Nurse.8. Report incidents and accidents to the Lead CCCT Nurse. Have an understanding of risk assessment management.9. Acknowledge the parameters of the role and refer any issues beyond the scope of the role to the On Call/Lead CCCT Nurse.10. Ensure a safe environment and safe use of equipment.11. Ensure fully conversant with emergency procedures.12. Participate in and complete an orientation and on-going training program comprising face to face and online education, ensuring that competencies in specific clinical procedures set out in protocols relevantto the child or young person are achieved and maintained.13. Maintain clinical knowledge and skills. To keep up to date with changes in practices and make full use of relevant opportunities for self-development.14. Attend mandatory training and safeguarding supervision sessions.15. Attend planned clinical supervision sessions and seek support when required. Frequent contact with families with complex health needs requires strong interpersonal skills and a high degree of professionalism, emotional resilience. The ability to ask for support is imperative when dealing with distressing and disturbing situations.16. Provide ad hoc support to the Childrens Outreach Nursing Service.

Carry out basic maternal observations of temperature, pulse, blood pressure, SOS scoring, blood glucose monitoring (once Trust training attended and Trust competency completed). And urinalysis, documenting them accurately in the womans maternal records and reporting deviations from the normal to the midwife. Obtain maternal blood samples on request in line with trust policy and procedure. (once attended Trust Venepuncture training and completed Trust competency) Provide emotional support to woman and birthing partner during labour as required. Provide basic maternal post-operative care including, basic wound care Carry out neonatal observations of temperature, respiratory rate, colour and weight documenting them in the appropriate records and reporting any deviations from normal to the midwife or an appropriate person i.e. GP or hospital doctor. Assist women with breastfeeding and other methods of feeding (infant cup feeding) as required following appropriate training and in line with local policy Provide education and support to mothers with breastfeeding issues within the hospital and community, in line with plan of care. Carry out New-born Blood Spot Screening following discussion with the parents and in line with policies and procedures.
