The major duties and responsibilities include but are not limited to:
- Work assignment review and oversight of program administrative and personnel management functions relative to the staff supervised.
- Responsible for the Social Workers assigned to the following areas: Community Living Center (CLC), Acute Rehabilitation Unit (ARU), Home Based Primary Care (HBPC), Hospice and Palliative Care (HPC), Palliative Care Consult Team (PCCT), Adult Day Health Care (ADHC), Geriatric Evaluation and Management Program (GEM), Community Residential Care (CRC), Contracted Nursing Home (CNH), Blind Rehabilitation Center (BRC), Medical Foster Home Program and the Traumatic Brain Injury (TBI) Clinic.
- Establishes guidelines and performance expectations for staff. Observes workers' performance; demonstrates and conducts work performance critiques. Provides informal feedback and periodically, at mid-year, and annually evaluates employee performance.
- Assures staff are trained and fully comply with the provisions of the safety regulations.
- Responsible for furthering the goals of equal employment opportunity (EEO) by taking positive steps to assure the accomplishment of affirmative action objectives and by adhering to nondiscriminatory employment practices in regard to race, color, religion, sex, and affirmative action for the area under their supervision in the following: (1) merit promotion of employees and recruitment and hiring of applicants; (2) fair treatment of all employees; (3) encouragement and recognition of employee achievements; (4) career development of employees; and (5) full utilization of their skills. Deals effectively with employees and union representatives on employee suggestions, complaints, grievances, and other matters involved in the day-to-day administration of the labor management agreement.
- Performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.).
- Develops and implements information systems to track service activities including visits, referrals, patient demographics, equipment utilization, housing, and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
- Responsible for submitting reports as required by the Medical Center, the VISN, and Central Office. These reports include monthly and annual reports as required.
- Administratively responsible for coordinating services between assigned programs to reduce barriers to care and increase access across multiple care lines and locations.
- Provides program oversight and monitors programs to ensure programs meet The Joint Commission (T JC) and Commission on Accreditation of Rehabilitation Facilities (GARF) accreditation requirements; ensures programs align with agency mission and strategic goals; initiates and leads performance improvement activities; and ensures programs incorporates stakeholder feedback.

Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the applicant or others. Position requires walking throughout the Medical Center and clinics, community agencies/court rooms, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. The incumbent must possess current driver's license and will be certified as an incidental driver, pursuant to VHA Directive 2008- 020.
