The Medical Specialty Care Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal specialty care. The focus is on individual growth and development in practice with increased levels of self-direction. This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Ability to formulate a plan of care based on nursing assessments and Veteran goals of care. Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. The nurse brings principles and theory with limited application of critical thinking skills.
The Medical Specialty Care RN coordinates nursing care within a team environment for patients within the medical specialty clinic setting. The Medical Specialty RN uses the nursing process to screen patients for initial and recurring provider appointments as well as assist providers with minor in-office procedures. The RN educates patients and family regarding the disease processes, medications, procedures, or treatment and equipment use. The Medical Specialty RN responds to patient phone calls regarding their needs and follow up care and provide education to patients/families. The RN administers, initiates, completes and/or follow up nursing referrals; documents and reports significant patient information; uses patient care equipment in a safe and appropriate manner and assures proper maintenance of Reusable Medical Devices (RMD); provides patient education and correctly documents findings. The RN provides patient education and correctly documents findings; orders and stocks supplies for the clinic and assists with the revision of SOP and medical center policies as needed. The Medical Specialty RN must have a general knowledge of medications and treatments commonly used in their discipline, knowledge of community resources specific to their patient needs, knowledge of the consult and referral process, and provision of emotional support to patient and families. The Medical Specialty RN appropriately and accurately documents in the Electronic Medical Record (EMR), participates in education events, and performs other duties as assigned by the Nurse Manager. They provide medication and education to patients as directed, triage phone calls, voicemail messages, and My Healthy Vet secure messages. They perform related duties as determined by the attending provider. With in the Interventional & Specialty Care Service, the RN will float to other specialty areas as needed and is required to have basic knowledge and competency in those area. The RN provides outstanding customer service to the patients they serve. Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans.
