The following are the duties of this position at the full working level.

The Physicians (MD/DO) in Primary Care (PACT) is accountable for adhering to established policies and standards of care/standards of practice in evaluating, tracking, and following up on patient care. The MD/DO works in collaboration with heath care professionals to provide longitudinal case management of the PCP's entire PACT panel with independent care delivery for acute/same day needs, or when otherwise requested by the PCP.

The Physician, Patient Aligned Care Team duties include, but are not limited to the following:

• Provide independent care delivery for chronic, acute/same day medical needs. The type of care is served to the Veteran population, with diseases consistent with those seen in the general population namely diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, mental health disorders, etc. A part of your responsibility is to write orders for diagnostic testing, prescribe medications, consult with specialty service, and utilize computerized/electronic medical records. Major function of the position includes Health risk Assessment, goal setting for prevention, and chronic diseases management. A major focus is in preventive medicine with focus on the achievement of positive outcomes for patients, improve coordination of care, decrease clinic waits or delays, and appropriate use of resources.
• Provide direct patient care to include, but are not limited to: history and physicals, medication management, evaluation of diagnostic tests and treatment planning, clinical reminder completion as applicable.
• Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare. Oversee PACTs compliance to Message Manager, MyHealtheVet, and encounter completion. Proficient in Tele-Health to home visits (VA Video Connect), and Comprehensive Women's Health care.
• Document all medical information in an electronic medical record.
• Perform medication reconciliation.

Functions or Scope of Assigned Duties:
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.-

1. Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.-
2. Develops and implements an individualized plan to achieve optimal population health and disease management.- Demonstrates leadership and works collaboratively with the interdisciplinary team
3. Orders and interprets labs, tests, and diagnostic studies.-

Duties included:
(1) Provides quality patient-centered care to adults age 18-65 and older adults age 65+, as assigned by ACOS of Primary Care.
(2) Assesses Veterans by utilizing advanced clinical judgment and physical assessment skills. Identifies acute/non-acute/chronic health problems.
(3) Ensures that safe, efficient, and high quality healthcare is delivered within the department.
(4) Ensures appropriate initial evaluation, treatment, and consultation, as well as documentation, consistent with current VHA Directive
(5) For admissions to the inpatient units, will ensure a physician-to-physician conversation regarding the need for admission, including diagnoses, assessment and recommendations.
(6) Demonstrate sound decision making skills, both clinical and administrative, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians.
(7) Daily care of patients also entails regular interaction with nursing, coordination of consultations and care and facilitation of diagnostic studies and therapeutic procedures.
(8) Other vital roles include: communication with patients and their families, patient education, discharge planning and communication with outpatient providers, arranging follow up, preparing discharge instructions, medication reconciliation, and ensuring communication during hand-offs of care.
(9) Ensures a sign-out to the oncoming physician to ensure an informed transition of care; coordinate with the admitting physician and consultants to organize diagnostic and treatment plans for all patients pending admission.

• Abstracts all necessary information and assigns codes (ICD-10, CPT, & HCPCS). Determines the final diagnosis and procedures stated by the physician are valid. Works with automated records systems. Assist with the review, analysis, technical evaluation of the Patient Component Encounter (PCC) form in relation to the completeness of diagnoses, procedures, and provider signatures.
• Conducts quality control and improvement tracking and identifying inadequate documentation. Communicating program software and hardware programs and maintaining workload logs. • Surveying and observing avian populations, looking for abundance, behavior, habitat, sex and age structure, and seasonal movements.
• Assisting in collecting data on hatchery populations to study health and propagation methods.

• Maintains involvement with the Rabies Project and the Mesocarnivore Genetic Archive to help understand the evolution of wildlife rabies and mesocarnivore populations in North America.
• Develops methodologies to assign emerging wildlife rabies cases to population or region of origin.
• Coordinates submission, receipt, and management of hair, blood, and tissue samples from mesocarnivores that are collected from across North America for the Mesocarnivore Genetic Archive.
• Develops standards for loaning of samples to other research institutions/individuals.
• Archives samples and the maintenance and disbursement of supplies and sampling strategy to field crews collecting mesocarnivore samples.
• Gathers and analyzes relevant genetic data from the archive; using sophisticated genetic statistical techniques, technologies, and equipment for research.
