Included but are not limited to:
- In-home primary care visits with medically complex older Veterans
- Lead interdisciplinary team in care planning for Veteran panel
- Medication management and oversight
- Eliciting patient and family goals and tailoring medical care to those goals
- Reviewing and updating care plans at least quarterly
- Covering for other providers for leave
- Coordinating care with specialty and home care providers within the VA and in the community
- Must have valid drivers license and ability to drive government cars in all weather
- Work collaboratively with experts from many disciplines
- Participate in Quality or Process Improvement projects
Work Schedule: Monday - Friday 8:00am - 4:30pm

- This position is in the Excepted Service
- Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held.

Pay: Competitive salary, regular salary increases, potential for performance awards
