Full Job Description

• To undertake the full range of offender management tasks with offenders assessed as low or medium risk of harm and to support the Probation Officer grade in high risk cases.
• When providing case manager support, to contribute to the delivery of the Risk Management plan and report significant changes relating to risk of harm and/or of reoffending or any non-compliance within agreed enforcement procedures
• To use computer based systems to produce, update and maintain records and other documentation within agreed timescales
• Ensure effective referrals to services and facilities and communicate with offender management staff, interventions staff, service providers and external agencies to review progress and associated risks.
• To undertake prison, home or alternate location visits as required in accordance with service procedures and policies.
• To undertake work in the court setting, including the completion of appropriate reports on cases and prosecution of breaches.

Essential Requirements

• Experience of working and supporting a diverse range of people who have experienced a range of social/personal difficulties, including working with groups and individuals to motivate and change behaviour.
• Experience of making effective referrals to and managing relationships between external agencies to enhance positive outcomes, manage risk and reduce reoffending.

An opportunity has arisen to become part of a dynamic trustwide specialist team. This will involve the care of Skin patients on the cancer pathway, in addition to supporting them living with and beyond a cancer diagnosis. The post holder will be an integral part of the nurse specialist team and compliance team, aiming to enhance patient experience through service delivery and development.





Under the guidance and supervision of a registered practitioner, the post holder will support the care of Skin patients and their family, during their cancer journey through diagnosis, treatment and follow-up, into living with and beyond cancer.

This role involves provision of emotional and informational support, demonstrating respect of patients’ diverse cultural backgrounds, whilst ensuring their privacy and dignity is maintained. This will involve telephone support of patients and their relatives/carers.

The role will involve supporting the Skin clinical nurse specialist team and the cancer compliance team.

The postholder will be required to provide administrative support and record-keeping, navigating and coordinating pathways, clinic maintenance and audit of nurse-led clinics, and ensuring clinic letters, prescriptions and appointments are relayed to patients in a timely and efficient manner.

The role will require liaison and effective communication with all members of the multi-disciplinary team, patients and carers to provide co-ordinated high-quality patient-centred care.

The post holder will play a key role in supporting the specialist nursing team to include providing evidence-based information and sign-posting to appropriate resources and completing of Holistic Needs Assessments.

Clinical

1. Coordinate the care for a defined group of patients assessed by a registered practitioner as having care needs suitable for supported self-management

2. Coordinate any necessary assessments, investigations and appointments to:

a) Coordinate the monitoring process and facilitate feedback of results for non-complex cases

b) Coordinate the timely escalation of processes / the pathway for those with complex results from remote surveillance and monitoring investigations



3. Be able to refer and sign post to the clinical team as appropriate for complex decisions

4. Provide basic telephone advice where appropriate and sign post to other sources of support

5. Support and advise people in accessing appropriate information, by sign-posting to a variety of resources. Promote and educate towards a self-management approach, where appropriate.

6. Deliver patient-centred, self-management support and education as necessary, to non-complex patients

7. Document and monitor all aspects of input, care coordination and service delivery, supporting data collection for audit

8. Contribute to Holistic Needs Assessments and development of an individual Care Plan in line with practitioner guidance, protocols and procedures

9. Implement, monitor and review the Care Plan with the patient and carer, in line with standard operating procedures and protocols and modify as appropriate

10. Organise and prioritise the designated workload in relation to identified needs

11. Evaluate outcomes of care delivery with the registered practitioner

12. Coordinate and support, with a team approach, the Skin PTL patients

13. Coordinate handover with other teams, to facilitate safe and effective transition of care between services in order to provide seamless support for people

14. Support the role of the registered practitioner/clinical team to ensure high quality patient centred care delivered

15. Communicate and work collaboratively with all healthcare professionals both in the Hospital and Primary Care setting, including support agencies

16. Communicate directly with patients and carers demonstrating empathy, understanding, diplomacy, honesty and integrity

17. Support the planning, delivery and evaluation of a variety of self-management initiatives in collaboration with the relevant clinical teams and wider stakeholders. This may include participation in assessments, clinical workshops or health and well-being education-based events

18. Carry out general administration duties as required by the role

19. Act as an advocate and facilitator to resolve issues that may be perceived as barriers to care

20. Support and contribute to audit processes, governance, research and service development as deemed appropriate

Administrative

1. Provide administrative support to the Skin Cancer Service

2. Ensure Holistic Needs Assessments are entered onto the relevant IT system and patient Care Plans are created and given to the patient

3. Support the implementation of Stratified Pathways for invasive skin cancer patients and once implemented, maintain data input for navigating patients on this pathway

4. Provide support to the skin Clinical nursing for gathering of audit data and patient experience feedback

5. Collaborate and work closely with local providers of care and resources, including information resources; signposting and referring patients, as and when necessary, to relevant support services

6. Evaluate the range of channels used to provide information/ resources and review with all stakeholders, the need for further developments

7. Be responsible for ordering any relevant supplies and equipment

8. Input/record accurate data to enable reports to be produced on the outcomes of self-management interventions and resources

Professional

1. Communicate effectively with relevant MDT’s and clinical teams to ensure that all appropriate cancer patients are identified for co-ordinated support, as assessed by health professionals

2. To work within agreed policies of East Kent Hospitals University NHS Foundation Trust

3. Work wtih the Trust wide Skin Cancer team in gaining patient experience feedback, communicating effectively with patients /care givers to aim to develop the service.

4. To be accountable for his/her practice and takes every reasonable opportunity to sustain and improve his/her knowledge and professional competence to include mandatory training as specified by East Kent Hospitals University NHS Foundation Trust

5. To be responsible for maintaining a personal portfolio to demonstrate professional development and training

6. To actively participate in any regional specialist meetings/groups to facilitate collaborative developments relevant to the role

Education and Development

1. Contribute to clinical/IT teams reviews of implementation issues of the remote surveillance systems and support and contribute to solution identification and resolution

2. To be confident with use of IT software to support patients in using this to access Holistic Needs Assessments and stratified pathways

The Elderly Care team with 14 funded Consultant Geriatrician posts (13 in place) currently provide a wide range of hospital and community based services including Acute Frailty services as well as running specialist clinics for patients with Parkinsons disease, falls and general medical conditions. We have recently started a Rapid Access Clinic for Elderly as part of Frailty- Same day Emergency Care (F-SDEC). Inpatient elderly care wards include 5 acute elderly care wards and an established purpose built Acute Frailty Unit with10 Assessment beds, 22 short stay beds and 2 clinic rooms. In addition to existing services, we are seeking colleagues to take on the challenge of developing new models of care for frail elderly people in the community such as Frailty. Pioneering work has already established Integrated Locality Review meetings (MDT meetings, 4/week). Consultants are working closely with existing community teams to proactively assess and manage frail individuals in the community. We are about to open a 22 bedded Acute Unit based in a Community Hospital in Sheppey. The post holder will join the existing team of full time Consultants. The post holder will need to take responsibility for the management of patients admitted to his/her specialty Elderly Care ward (12-15 patients) routinely and those admitted under their care whilst on call for Acute Frailty. The latter would average 12-15 per on call. These patients are then streamed to Frailty Short Stay Unit if early discharge planned (<48 hours) or one of the 5 Frailty Base wards for longer admissions. Acute Frailty Unit has a dedicated Elderly care Team with 2 Consultant Geriatricians supported by their own team of junior doctors and a Multidisciplinary Team. There are 10 funded posts for Middle Grades in the department with currently 6 in post. Elderly care Consultants participate in an Acute Frailty On call service. The post-holder will be paired with another Consultant so that in a 12 week block they provide 2 weeks of weekday on call and 2 weekends on call. Full registration with the GMC and the ability to make decisions at a senior level, the role will allow you the opportunity to continue your personal development, and participate in the teaching, research and governance programs within the Department. All subspecialist interests are welcome. There is an opportunity, in negotiation with the other consultants, for participation in the running of the established clinics such as Falls and Movement Disorder Clinic. The successful applicant will also have an outpatient workload comprising of new and follow referrals for managing complex co-morbidities. Typical clinic would have 3-4 new patients and a similar number of follow-up patients.
