The Advanced Medical Support Assistant (MSA) in Primary Care works collaboratively in an interdisciplinary coordinated care delivery model and performs receptionist duties, customer service and other duties assigned for the proper and timely treatment of patients and maintains appointment schedules for one or more outpatient clinics. Patient care is delivered in a team based model known as the teamlet. S/he works as a member of the teamlet including a Registered Nurse Care Coordinator, a Licensed Practical Nurse/Licensed Vocational Nurse and a Primary Care Provider. The teamlet provides the majority of the healthcare needs for an assigned panel of patients. The teamlet works collaboratively with other members of an expanded health care team including pharmacists, social workers, dieticians, behavioral health staff, etc. to provide a robust interdisciplinary approach to care. Duties may include but are not limited to:

• Incumbent must screen/receive phone calls in courteous and timely manner, determine the nature of requests and provide the information desired using privacy rules and established clinic processes. A caring, compassionate, and sensitive manner is essential when explaining procedures to veterans and their family members both in person and on the phone.
• Incumbent is responsible for identifying incomplete encounter information and communicating findings to providers to facilitate accurate workload credit and revenue collection.
• Incumbent is able to independently manage position responsibilities with little supervision as well as set priorities and deadlines adjusting the flow and sequencings of the work to meet team and patient needs.
• Incumbent provides administrative support to Physicians, Physician's Assistants, Nurses, Advanced Registered Nurse Practitioners, Dietitians, Social Workers, and other staff throughout the Primary Care Service area.
• Incumbent participates in the daily huddles and team meetings where patient care planning and management occur, except during staff shortages for coverage.
• Incumbent notifies supervisor when clinic access is less than desirable or if an individual patient cannot be scheduled within mandated clinic timeframes. Dailey review of active/pending orders and consults for accuracy and disposition. Coordinates administrative services for veterans, family members, caregivers, and general public, administrative and clinical staff to ensure continuity of inpatient and outpatient care.
• Incumbent will ensure that when records are received, all necessary health/administrative information is integrated into Computerized Patient Record System (CPRS) by sending the documents to scanning.
• Develops and maintains specific reports used by the team to monitor, track and trend care delivery.
• Develops and maintains effective working relationships with all clinical and administrative staff in determining priorities of activities in support of direct patient care.

Duties include:

• Issuing backcountry permits and maintains the backcountry reservation system.
• Accurately describing camping areas at high elevations and remote locations.
• Making visitors aware of general and specific mountain safety hazards.
• Welcoming and orienting visitors to the park setting.
• Responding to visitors questions concerning the full range of resources, facilities, services, and activities.
• Handling government monies associated with selling and remitting boating, entrance and backcountry reservation fees.
• Ensuring accountability so that shift reports are properly filled out. Performing audits and remits.
• Training new employees on the operation of permit/fee computer programs and all permits operations including issuing backcountry permits and trip planning for remote backcountry terrain.
• Patrolling the backcountry for the purpose of learning the trails, rivers and lakes to inform the public of trail and remote backcountry conditions.

The Surgical Specialty Care Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal specialty care. In this first year, the novice is "taught about situations in terms of objective attributes and other measurable parameters of a patient's condition, features of the task world that c an be recognized without situational experience. While a novice brings principles and theory, context-dependent judgments and skill can only be acquired in real situations" (Benner). The focus is on individual growth and development in practice with increased levels of self-direction. This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Ability to formulate a plan of care based on nursing assessments and Veteran goals of care. Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. The nurse brings principles and theory with limited application of critical thinking skills.

The Surgical Specialty clinics require specialized nursing care and evaluation depending on services provided. The RN is responsible for the surgical and non-surgical patient care while utilizing all aspects of the nursing process and works in collaboration with the care coordinators, providers, residents, surgical technicians, and nurse manager. The RN systematically assess the health status of individuals and documents related health data in accordance with institutional policy providing pre, intra, and post procedure care. Additionally, the RN provides education to patients, families, significant others, and/or caregivers. The RN may be required to provide post-op care, wound care, or any other nursing interventions as permitted by governing licensing authorities and/or the Veterans Health Administration.

The Surgical Specialty RN is required to train and maintain competency in set-up and break down of minor procedures. The RN will float to other surgical specialty areas as needed and is required to have basic knowledge and competency in that area (to include: GYN and Procedures, Orthopedics, ENT, and Urology/GU Clinics). The RN will enter orders for labs, supplies, and prosthetic items as permitted by nursing protocols/policies. The Surgical Specialty RN performs initial triage of walk-in patients to the clinical area that may need urgent or post-operative care on a daily basis. The RN will determine if the patient requires further intervention with provider support, to improve same day access. The clinic RN will administer medications, perform treatments according to policy and observes for changes in the patient's condition from ordered medications/treatments and reports said changes immediately. The clinic RN must possess the ability to recognize urgent/emergent care situations and perform appropriate interventions. The RN must complete phone calls and secure messages within 24-48 hours and reviews alters throughout clinic hours and resolve promptly, and ensure the clinic has an efficient flow and education to team members to meet clinic/patient needs. This RN must also understand tele-health medicine to include VVC, CVT, SM and its equipment and quality monitors as assigned by nursing leadership.

Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans.
