We are a Brief intervention service providing short term treatment, care and support for people with mental health difficulties in the primary care sector. This role will give you the opportunity to network , build relationships with services & patients from across the Primary Care Network.

The successful candidate will work closely with our new Primary Care Network Mental Health Team to facilitate reviews of patient needs and initiate mental health treatment for a range of conditions. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a one workforce cross system approach to the onboarding, induction, education and experience of Healthcare Assistants. To support this one workforce model the successful candidate for this post will be required to attend our5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. The Specialist Dementia and frailty teams provide a comprehensive assessment, diagnostic and treatment pathway supporting patients of any age with dementia as well as older adults over 70 years of age, who have functional mental health needs with an associated age related frailty.
