• The Palliative and Hospice Care Program Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of the palliative and/or hospice patient.
• He/she is accountable for adhering to established policies, standards of care and standards of practice and for demonstrating leadership and accomplishments when providing patient care and in developing and implementing programs to improve the delivery of patient care.
• The incumbent screens patients for the medical necessity, and provides appropriate care for veterans, taking into consideration their diagnosis, and physical, psychosocial, spiritual, and cultural needs.

General Description of Assigned Duties;

The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or
otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses).

Specific Duties;

The incumbent is accountable to the Chief Nurse for Community Care. The incumbent is a registered professional nurse, qualified by education and experience. The incumbent accepts responsibility for assessing, developing, implementing, evaluating and revising the program elements performed by the Community Care staff. He/she coordinates with Care in the Community staff, VAAAHS providers, non-VA providers, and non-VA facilities regarding program elements. These elements include outpatient non-VA care, inpatient non-VA care, authorizations, claims review, appointment scheduling, and other Community Care program elements. The goal of Community Care is to facilitate high quality, appropriate, and cost-effective care for Veterans receiving non-VA care. Clinical oversight of the non-VA care processes is provided by the Chief of Staff and/or designated appointee. He/she promotes, creates, demonstrates and ensures a culturally diverse workforce in which the values and needs of all individuals are respected. The duties of Community Care Nurse Manager are considered a non-bargaining unit assignment, since the duties involve supervision of assigned personnel and engagement in administrative work, in other than a purely clerical capacity. The Community Care Nurse Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of patient care for adult and geriatric patients, in accordance with the Michigan Nurse Practice Act, Medical center, and VA standards of care. The Community Care Nurse Manager has continuing responsibility for all the following functions: (1) planning work to be accomplished by subordinates, setting priorities and preparing schedules for completion of work; (2) assigning work to subordinates based on priorities, selective consideration of difficulty and the requirements of the assignments, and the capabilities of subordinates; (3) evaluating the performance of subordinates; (4) making recommendations for appointments, advancements, or reassignments of subordinates; (5) giving advice, counsel, or instruction to subordinate personnel on work and administrative matters; (6) hearing and resolving complaints of subordinates and referring more serious complaints not resolved to higher level supervisors; (7) recommending and/or taking disciplinary action where appropriate; and (8) identifying developmental and training needs of subordinates and providing or making provision for such development and training (VA Handbook 5007, Part III, Chapter 8). The Community Care Nurse Manager is also responsible for performance measures, performance improvement, customer service/patient satisfaction, patient and employee safety, interviewing applicants, employee relations, standards of care and practice, the work environment, and accreditation and regulatory standards. The Community Care Nurse Manager is responsible for time scheduling of the unit, coaching staff, dealing with employee conduct and performance issues, completing and submitting recurring reports, proficiencies and performance evaluations in a timely manner. The Community Care Nurse Manager participates on service and medical center committees and task forces to improve nursing practice and patient care.

MAJOR DUTIES AND RESPONSIBILTIES

Technical Program Duties: 65%

• Under general supervision, the Program Support Assistant (PSA) is responsible for the submission of all statistical and administrative reports or data from their designated assignment area with the aforementioned programs. This may include data research and entry, program reports, patient lists, accounting and budgetary duties and ongoing responsibility for projects. problems. or requirements that arise within their designated work site. The work will be performed in a timely, professional and courteous manner to sustain quality patient care and customer satisfaction.
• Responsible to operate all necessary applications within FBCS, CPRS and VistA (including but not limited to VistA FEE, Lodging, Admissions, AOD, Caregiver, TCM, MFH and Eligibility Computer Systems as well as ability to work with Microsoft Word, Outlook, Power Point, SharePoint and Excel Spreadsheets).
• Will assess the dimension, consequences, and impact of the assigned work and thus prioritize, collect, and assemble the necessary facts, resolving conflicts, and performs these functions independently. Creativity in the development of procedures where no guidelines exist may be necessary; will identify these irregularities in the course of the work and reconcile those problems .
• Educate Veterans and family members of Veterans on benefits, eligibilities alternative lodging and other available programs with in the Service. Issue discount letters for non-eligible or families, meal vouchers as appropriate by policy.
• Monitor and ensure external supporting services goals are met i.e. dietary, housekeeping, logistics, respiratory and voluntary to complement our systems or services.
• Resolve problems with office equipment malfunctions, obtaining JRM, Engineering, copier/fax technician, or Facilities Management support as indicated.
• Assigned as an authorized user of the Government Purchase Card for small item purchasing in their designated site and section. As an authorized user of this card the incumbent is responsible for meeting the standards for documentation and monthly reconciliation of the purchase cards in accordance with all rules and regulations.
• Automated Data Processing Application Coordinator (ADPAC) liaison, responsible for systems administration within the service and will provide guidance and assistance to clinical staff within the service. Conduct training for new employees as to the administrative requirements of their jobs in the program, i.e., function, documentation, clinic encounters. etc.

• Works with a patient population ranging from age 18 through senior adults (age 65 and over) and must be aware of, and respond appropriately to the physical, social, and psychological differences among these groups.
• Communicate effectively with patients, patient's family or caregiver, clinical staff, and hotel contractors in person or by telephone in order to assess the Veteran's needs.
• Take messages; track call-ins/call-off by other staff members, and notify the appropriate supervisor, they will assess Veterans' needs, assist with the Veteran's situation or redistribute to the appropriate manager, social worker, department, patient advocate, etc.
• Handle consult requests expeditiously or assign to the respective social worker as they are received in the service and log them into the tracker.
• Working with the clinical unit/providers, the PSA will ensure Veterans are screened and approved appropriately prior to social services being rendered.

• Obtain, organize, file and retrieve reports, correspondence, and numerical data for use in preparing status reports, briefings, and presentations i.e. daily AOD log, gain and losses sheets, lodging lists and admission reports, medical records etc. in accordance with all Federal, VA and local policy and guidance.
• Responsible for the administrative handling of all records, forms and documents, retrieval and dissemination of data relating for the programs listed above.
• Type, edit, maintain current copies of records or forms relating to the program such as letters, memos, reports and summaries, surveys, program brochures, letters of introduction, and other types of written communications.
• Compose correspondence; prepare agendas and meeting minutes as indicated to assist clinical functions of as the need arises.
• Print daily roster of Veterans when necessary, maintain a weekly list of scheduled CNH Veteran VA medical center appointments and arrange travel consults to meet the appointments.
• The PSA will perform other duties as assigned.

• The incumbent carries out required performance of this position as transportation program support assistant Guest Relations Program to the Office of Veteran Experience. The program is designed to provide Veterans/patients, their families and visitors with staff who can respond quickly to inquiries, identify problems, and improve the experience of those visiting the campus. The services are designed to respond to customer immediate needs including navigation, direction, and information assistance.
• As a key challenge of patients, navigating the campus will be a primary focus along with providing a positive first impression. For those who do not know how to access medical center programs and/or services, personal assistance will be provided by the staff. The incumbent serves as the initial, as well as the final point of contact for guests entering and leaving the facility and is a key influence toward patient/public perception. In this role, the incumbent performs these responsibilities using skills that will foster a culture of care at the VA facility. The incumbent is assigned to the Office of Veteran Experience and Employee Engagement.
• The incumbent performs customer service guest relations, shows understanding, friendliness, courtesy, tact, empathy, cooperation, concern, politeness respect and dignity to others; relates well to people from varied backgrounds and different situations. He/She receives, greets and direct callers over the telephone and in person with precise clear instructions. The Physicians (MD/DO) in Primary Care (PACT) is accountable for adhering to established policies and standards of care/standards of practice in evaluating, tracking, and following up on patient care. The MD/DO works in collaboration with heath care professionals to provide longitudinal case management of the PCP's entire PACT panel with independent care delivery for acute/same day needs, or when otherwise requested by the PCP.

The Physician, Patient Aligned Care Team duties include, but are not limited to the following:

• Provide independent care delivery for chronic, acute/same day medical needs. The type of care is served to the Veteran population, with diseases consistent with those seen in the general population namely diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, mental health disorders, etc. A part of your responsibility is to write orders for diagnostic testing, prescribe medications, consult with specialty service, and utilize computerized/electronic medical records. Major function of the position includes Health risk Assessment, goal setting for prevention, and chronic diseases management. A major focus is in preventive medicine with focus on the achievement of positive outcomes for patients, improve coordination of care, decrease clinic waits or delays, and appropriate use of resources.
• Provide direct patient care to include, but are not limited to: history and physicals, medication management, evaluation of diagnostic tests and treatment planning, clinical reminder completion as applicable.
• Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare. Oversee PACTs compliance to Message Manager, MyHealtheVet, and encounter completion. Proficient in Tele-Health to home visits (VA Video Connect), and Comprehensive Women's Health care.
• Document all medical information in an electronic medical record.
• Perform medication reconciliation.

• Nurse Manager (NM) is the designated first-line supervisory authority and is responsible and accountable for the 24/7 operation of their assigned units and provides coverage for other NMs as needed. The NM is responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance. They function as a role model, educator, practitioner, and supervisor providing leadership to the team. They enhance care outcomes by collaborating with members of the interdisciplinary team to remove barriers and implement new evidence-based cutting-edge programs/processes.

• The NM maintains knowledge and ensures compliance with all regulatory and accreditation standards, Directives, policies, and standard operating procedures (SOP).

The Supervisory Medical Support Assistant will perform administrative support duties that may be performed in a Clinic Based Outpatient Clinic (CBOC), Inpatient or Outpatient Clinic. The Supervisory MSA at this level plans and directs programs at a medical center and/or satellite outpatient clinic and is delegated full supervisory responsibility. Assignments at this level include but may not be limited to: assigning and evaluating the work of subordinate staff; resolving complex problems to ensure that patient services are met; evaluating new products, equipment, and systems in order to make recommendations for improved operations; identifying educational and training needs; making final decisions on selections; evaluating performance; and taking disciplinary action when necessary. The Supervisory MSA is delegated full administrative responsibility for planning and directing the MSA's activities. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) will provide a full range of nursing care to Veterans/Patients, including complex care needs using the nursing process.

• Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department

The incumbent assists in all aspects of the development, coordination, and management of the Center for Development and Civic Engagement (CDCE) Office and is responsible for independent management of specific programs or portions of larger programs. This includes, but is not limited to, planning, establishing and implementing policy, developing program goals and objectives, managing, evaluating, and coordinating (internal and external) resources of assigned areas.

• Assist in Development, coordination, and management of Center for Development and Civic Engagement (CDCE) office.
• Takes proactive role in meeting the needs of the facility or other VA affiliated program in establishing a vital link with the community.
• Interviews, schedules, places, and evaluates volunteers.
• Prepares volunteer personnel files in accordance with The Joint Commission and standards of VHA volunteers.
• Coordinates data analysis from diverse sources to prepare reports and evaluations of CDCE/Voluntary service programs and other initiatives.
• Plans and executes a variety of complex internal and external special events, including VISN-wide and national events where the local facility is a key host or sponsor, or responsible for securing major monetary or in-kind donations and volunteer support.
• Implements the strategic and operational plans developed by the CDCE/Voluntary Services Officer.
• Develops recruitment materials geared at both volunteers and community partners.
• Manages implementation of the Continuity of Operations and Emergency Management Plans in relation to volunteer training and communication, deployment of resources , and support of the overall facility emergency plan.

MAJOR DUTIES include but are not limited to:

• Set up assigned station with the correct supplies and food items
• Serve food cafeteria style by placing uniform portions of food on customers'/patients'/residents' plates
• Break down and clean the station when assigned
• Set up dining room tables for service, place food and beverages on tables, and replenish items as necessary
• Return soiled trays and dishes to the dish room
• Deliver meal trays to the patients' bedsides and report the patients' comments and complaints to the supervisor or dietitian.
• Prepare beverages according to the number of servings required
• Prepare fresh fruit/cold salads/dressings/sandwich fillings and cold sandwiches and simple cold desserts in specified quantities
• Sort, wash, peel and cut cold foods using knives or other equipment
• Provide assistance to cooks in the food preparation area, such as weigh, measure and assemble ingredients according to standardized recipes
• Prepare uncooked food items, such as sandwich spread and salad dressings
• Operate and break down and clean all equipment assigned to food service and related areas
• Portion food items into standard serving sizes using the proper utensils and specified dishware
• Prepare boxed/to go meals
• Select and place correct items on patient trays
• Read and interpret a regular or modified menu, individual diet cards, tray tickets or patient selections
• Identify obvious discrepancies between the prescribed diets and the food items designated by the menu
• Decide what food items to serve for the most common diets
• Perform heavy-duty cleaning tasks throughout the food service and related areas
• Separate food waste, trash/recyclables from dishes, glasses and silverware
• Unload food/tray delivery carts
• Store sanitized dishes, glasses and silverware
• Scrape, soak, scour, and scrub cookware and utensils
• Move garbage cans when collecting and transferring trash from the work area to the disposal area

The Extended Care Service Community Living Center Staff Nurse is a Professional Nurse with broad-based clinical knowledge enabling him/her to effectively implement health promotion, disease prevention, and management for adult or older adult residents requiring skilled Nursing interventions in a long term care setting. CLC Staff Nurses typically provide care for residents with complex, mixed chronic medical and/or psychiatric conditions, residents with cognitive impairments, and residents who are terminally ill. The CLC Staff Nurse utilizes the Nursing process when assessing, planning, implementing, evaluating, and documenting Nursing care provided. He/she works collaboratively with the interdisciplinary team when planning and providing care to Nursing Home residents and their families. The Staff Nurse functions independently as a charge Nurse on the unit, organizing and delegating care activities to other RNs, LPNs, and NAs and providing feedback to the Nurse Manager regarding the practice of others in order to improve care. He/she may either direct the care of other Staff members or provide direct patient care depending on the needs of the unit. The CLC Staff Nurse participates in Quality Improvement activities in the CLC and/or Medical Center with the objective of improving patient care outcomes. He/she takes an active role in identifying his/her own educational needs and continually updating clinical skills.
