Incumbent is responsible for assuring that various aspects of staffing, training, compliance, reporting, quality assurance, and budgetary initiatives are met. The RN Care Coordinator provides care coordination for veterans who are assigned to a Geriatric Patient Aligned Care Team (PACT). Coordination of care and services for veterans that includes but is not limited to veterans with chronic disease, veterans with limited functional status, complex medical, psycho-social and/or mental health needs, severely-injured service members and veterans, and veterans with intensive continuing care needs that will require coordination of and referrals to various levels of care. The incumbent begins the coordination process at the time the patient is identified and works collaboratively with staff within the Patient Aligned Care Team as well as staff throughout the Veterans Health Administration (VHA) facility that may also be involved in his/her care, staff from other VHA facilities, Military Treatment Facilities, and referring facilities/community agencies. The Mobile Provider iPad Program provides iPads to Clinicians to use in the day-to-day patient care activities. The AMSA:

The Medical Support Assistant (MSA) position serves as a Consult Coordinator for Community Care. The MSA works collaboratively with other members of expanded health care teams (i.e., Patient Aligned Care Team (PACT), specialty care) including Community Care providers, pharmacists, social workers, dieticians, behavioral health staff, etc. to provide a robust interdisciplinary approach to care. The Primary Care Provider (PCP) Float will provide primary care services in support of primary care operations in the San Antonio service area. As a float provider, this position will provide support to Patient Aligned Care Team (PACT) at an assigned San Antonio Clinic and may be detailed to other VA Clinics in San Antonio as staffing and workload dictate.

THE PCP Float may be asked to provide coverage for a vacant/absent PACT provider to supplement PACT services by delivering same-day care, routine scheduled care, or new patient intake for PACT panel(s).

The Primary Care Physician is responsible for providing Comprehensive Primary Care for Veterans and should, in the context of a longitudinal relationship, fulfill all primary care needs, including acute and chronic illness, gender-specific PC, mental health (Screen for conditions such as depression, substance use/abuse, military sexual trauma and PTSD, and evaluate and treat uncomplicated mental health disorders as well as substance use disorders and refer patients appropriately for further evaluation and treatment of sexual assault and military sexual trauma, mental health disorders and substance use disorders) and preventative care (age-appropriate cancer screening e.g., colon, breast and cervical cancer - as well as weight management counseling, Chlamydia screening (when indicated), smoking cessation and immunizations, etc.) along with coordination of care delivery (when specialty care is needed, the primary care provider will coordinate this care and communicate with the specialty provider regarding the evaluation and treatment plan to ensure continuity of care).

The PACT Model is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of health services through an interdisciplinary team that includes the PCP, RN Care Manager, LVN, clerical associate as well as other clinical services necessary to meet the health goals and needs of the Veteran patient including specialists. While the Primary Care provider's relationship with the Veteran patient will remain key, that bond will be augmented and strengthen by involving other health care staff as part of an integrated team. Together the team takes responsibility for the Veteran patient's ongoing care through a cooperative effort among all members.

Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status, or demographic status (e.g., women veterans, OIF/OEF veterans, employees, etc.). Typically these duties are designated at the time of joining but occasionally during the course of employment, participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives.

Physicians may be assigned duty as a supervising or collaborating physician for one or more mid-level providers. Physicians are expected to provide clinical oversight and review the care provided by mid-level practitioners. Most providers will need to act as surrogates for the patients of their colleagues, when one or more of their colleagues are on leave.

The PACT team is comprised of a provider, RN, LPN, and administrative support person. Although this position partners with other PACT team members, PACT LPNs successfully and consistently demonstrate independent technical judgment as well as comprehensive and thorough working knowledge of complex practices and procedures. The incumbent demonstrates knowledge, experience, skill, and self-motivation to carry out complex patient assignments, which include prioritization and coordination of clinical activities with minimal supervision. Duties include but not limited to:

1) Manages patient care problems and deviations using self-direction in accordance with VA Nursing Practice and LPN licensure standards. Maintains knowledge and skill to care for a wide range of patients, including those with chronic and acute illness. Demonstrates comprehensive understanding of disease conditions, nursing principles, and standards/practices to coordinate nursing care required in the clinic setting and alter the sequence and/or care delivered to meet individual patient's needs.
2) Assesses, records, and reports vital signs, neurological signs, weight, and blood glucose levels. Appropriately triages and prioritizes patient needs as routine, urgent, or emergent in the clinic setting and modifies patient care/treatment as needed.
3) Checks patients into the clinic, interviews patient and/or significant other, and documents intake data as per policy. Possesses comprehensive knowledge of VA protocols and VHA directives related to clinic management, documentation, and computer technology.
4) Demonstrates knowledge, skill, and ability to promptly recognize and effectively intervene in potentially urgent or emergent situations. Demonstrates the ability to independently adjust or modify care being delivered to meet the changing needs of the patient with notification to the RN or provider for concurrence. Responsibilities include prompt recognition and treatment of urgent/emergent patient care needs, including hypo/hyperglycemia, acute shortness of breath, acute chest pain, seizures, stroke symptoms, mental health issues, etc.
5) Maintains and implements knowledge of anatomy and physiology as well as scientific principles underlying nursing care and procedures. Provides high quality nursing care using traditional and non traditional modalities, including triage prior to provider face-to-face visits, nurse health maintenance visits, telephone triage, secure messaging, etc. Applies knowledge by considering the interrelationship between age, physical, emotional, social, cultural, and population-specific needs when providing care.
6) Active participant in quality improvement projects/hospital committees to promote evidence based practice and performance improvement in the clinic setting. Demonstrates engagement by regular attendance at committee meetings and providing committee updates during monthly staff meetings.

Incumbent is responsible for assuring that various aspects of staffing, training, compliance, reporting, quality assurance, and budgetary initiatives are met. These activities directly affect the revenue cycle, patient access to care, hospital performance goals and objectives, as well as VHA public image. The incumbent plays an integral role in identifying and developing ways to resolve problems with the administrative and clinical staff on access, customer service, patient flow, revenue, and data validation issues. His/her work also impacts processes, action plans, monitoring, and reporting. Major duties and responsibilities include (but are not limited to):

The Chief of SPS is responsible for administrative functions of the SPS program which includes membership on the following committees: Infection Prevention/Control; Water Safety Committee (Biomedical Engineering); Clinical Executive Council (CEC); Veterans Integrated Services Network (VISN) 17 Advisory Board; Clinical Product Review Committee (CPRC); Reusable Medical Device (RMD) Committee; Environment of Care (EOC) Rounds; and the Enterprise Equipment Request (EER) Committee. The Chief of SPS functions as an administrator, educator, and consultant utilizing management theory in collaboration with Patient Care Service (PCS) and other services. Establishes and maintains effective relationships with all levels of medical center personnel and relate effectively with customers, and the community as well as program and service leaders at the local and network levels. Key relationships with staff are needed to assist SPS managers and Lead Medical Staff Technicians (MST) to carry out the functions and activities expected of them, contribute ideas and recommendations for the establishment of standards of care outcomes, policies, and objectives for the enhancement of nursing under SPS, organization-wide, assist in policy-making activities as related to customer service and the overall functioning of administrative and clinical programs, and collaborate with other health care staff to establish and maintain programs that cross service and/or discipline lines and influence organizational mission and health care.
The Chief of SPS plays a leadership role in ongoing systematic evaluation and improvement of quality and effectiveness of SPS program outcomes. The incumbent provides health system-wide leadership for critical and semi-critical RMD, including sterile storage, reprocessing, standard operating procedures (SOP), competency assessment, selection, operation, and maintenance of reprocessing equipment, staff education program for RMDs, and quality management of SPS. The incumbent is responsible for assessing, monitoring, developing, and implementing corrective action plans for instrument processing, decontamination, sterilization, storage, biological monitoring, and high-level disinfection monitoring through an SPS Quality Management System. Additionally, the incumbent maintains oversight and availability for consultation of non-critical RMD. The incumbent assesses the environment, analyzes trends, transmits data, communicates ideas, develops, and implements policies, initiates programs and systems, and collaborates in the management of resources. The incumbent has knowledge in areas of patient care practices, microbiology, asepsis, disinfection/sterilization process, adult education, infectious diseases, communication, program administration and epidemiology. The incumbent has responsibility and oversight of evidence based practice (EBP) infection control relating to sterile processing standards. Specific activities include but are not limited to education program development, evaluating concepts and project development, mentoring staff and medical providers in sterile processing principles and in the implementation of projects, data collection techniques, statistical analysis, and developing evidence based clinical guidelines and protocols. This position is accountable for the SPS as well as oversight of the use of RMD within the VA Texas Valley Coastal Bend Health Care System and VA Texas Valley Coastal Bend Health Care System Community Based Outpatient Clinics (CBOCs).
