Successful candidates will work with patients to complete non-clinical elements of cancer care reviews and help patients understand why cancer screening is key for early diagnosis and support them whilst awaiting diagnostic tests for possible cancer and investigate those patients who fail to respond or attend for this. You will act as the patients coordinator between hospitals, community and other services and organisations. MAIN RESPONSIBILITIES A large part of your role will involve supporting patients on the cancer pathway. A cancer pathway is the patients journey from the initial suspicion of cancer, through clinical investigations, patient diagnosis and treatment. Support the practices in your PCN to improving local uptake of National Cancer Screening Programmes, understand the practices data for uptake on screening and understand how the practice can carry out the screening Supporting both patients and primary care to ensure that patients referred on a cancer pathway have completed all of the necessary investigations required set out in the 2 Week Wait referral criteria. Work closely with Trust colleagues to ensure that the patient attends the appointment. A cancer care coordinator will support patients at all stages in their cancer pathway, from primary care to diagnosis, undergoing treatment and beyond. Ensure that all patients are signposted to or receive information on their referral including why they are being referred, the importance of attending appointments and where they can access further support. Be available for patients with cancer, or their relatives, to ask questions and deal with problems that arise, linking in or signposting to services such as the hospital team, district nursing or Macmillan services, benefits agency as appropriate. Review patients and follow up each patient to complete the non-clinical elements of the cancer care review - following a cancer diagnosis, covering topics such as benefits, support groups, offering support to relatives. Be a point of contact for survivors of cancer, or bereaved relatives, who need support, signpost to support groups available locally or nationally as appropriate. Support the practices in your PCN in conducting peer to peer learning events that look at data and trends in diagnosis across the PCN, including cases where patients presented repeatedly before referral and late diagnoses. At Care in Mind our homes are always a hub of activity and energy where the young people can feel safe, and our staff enjoy coming to work. Your role will involve working with our young people to promote independence through support with education, household tasks and general life skills. You will also update online care records and complete medication administration. In addition, you will be responsible for the general upkeep and tidiness of the home ensuring the young people live in homely environment. At our home in Liversedge, West Yorkshire you will be working with young people who have a variety of mental health diagnosis including personality disorder, those who have difficult relationships with food and those who use self-harm as a strategy to cope with their emotions. As such, you will be required to be patient and resilient as the support you will be providing to our young people will include supporting them to improve their relationship with food and managing incidents of self-harm. When our young people are preparing for moving on positively from the home into the community, you will be supporting this transition through things such as: supporting them to self-administer their own medication, re-engaging with education, accessing public transport and appointments independently. The young people enjoy in house activities such as arts/crafts, baking, cooking, board games and quiz/movie/theme nights. They also like going out in the community, on day trips to the beach or similar, shopping, eating out, bowling, walks, concerts and the cinema. The home is a 5-bed service and is an Edwardian building with lovely spacious rooms including plenty of communal areas where young people and staff can work together.
