Provide excellent administrative support to the Frailty team. Communicate with patients face to face, via telephone, online and email, both in the community and within practice. Work with colleagues in the frailty team to identify patients who would benefit from proactive review. Work with patients, carers, and families helping them to organise appointments, arrange support and signpost relevant services. Work alongside a multidisciplinary team to ensure comprehensive and cohesive patient care. Liaise with community partners such as district nursing, palliative care teams, voluntary sector organisations, the integrated care therapy team to improve patient and carer outcomes. Summary of Post: The receptionist is the first contact between the patient and the practice. She should be pleasant, helpful and accurate in communication. The reception staff are responsible for ensuring the smooth running of the reception area. Key Elements of the Job: To maintain medical confidentiality. You will also facilitate the communication between other members of the Primary Health Care Team. You will be responsible for dealing with day-to-day general enquiries from patients. Clerical Duties As part of your duties you will be required the file and retrieve patients medical records, results and hospital letters. You will also be responsible for entering information on to the practices computer system, and retrieving information from the computer system. Key Tasks Answer telephone calls and deal with queries promptly, responding or taking messages as appropriate ensuring that an accurate, quality information service is provided. To manage the appointment system so that patients are able to see clinicians in appropriate time and manner. Signpost patients to the most appropriate service for their care To open and organise the distribution of the post To file information into patients notes using scanning where appropriate. To photocopy and file information To work flexibly with colleagues to cover the opening hours of the surgery, including opening and closing the surgery by arrangement. In accordance with the agreed PCN timetable, the post-holder will undertake a variety of duties, including: Conducting surgery and telephone consultations, addressing patient queries, and visiting patients at home. Checking, signing, and authorising prescriptions, while managing queries, paperwork, and correspondence in a timely manner. Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred by other health care workers. Attending MDT meetings Participation in education/teaching of wider MDT teams including medical and non medical teams Compiling and issuing computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible. Prescribing in accordance with the Practice prescribing formulary or generically, as clinically appropriate. Undertaking emergency patient visits during surgery hours, if clinically indicated. Participating in on-call sessions and home visiting sessions as allocated in the practice rota.
