We have a great job opportunity for you to join our newly formed ETAAS team, supporting customers to rebuild their lives after periods of street homelessness or a breakdown in their current accommodation. These include offending histories, substance misuse, mental health issues and learning disabilities.
Typical day as a Support Co-ordinator
• Creating support plans with your customers and co-ordinating a range of different services to support our customers within a multi-agency approach.
• Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation when they are able.
• Attending regular case conference meetings, ensuring the team is working collaboratively and utilising all additional support to give our customers the best chance of success.
• Carrying out risk assessments, support planning, goal setting and regular reviews.
• Cuppas, chats and catch ups. Communication and building relationships is essential and that takes time.
• Responding to customer emotional crisis and needs.

You bring
• Passion to support our customers to live their best life, working collaboratively with an eye for detail.
• Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
• Experience of co-ordinating and assessing customer referrals.
• The ability to work on your own initiative, remain calm under pressure and have a resilient approach, with firm boundaries and a consistent, creative style.
• A highly organised and practical approach, to deal with housing management queries, personal needs and you’ll also be confident to work with our contractors.
• Experience in safeguarding, risk assessments and confidentiality, working collaboratively with your colleagues across the organisation, and external agencies.
Our Team
You’ll join Calvin our Service Manager, who joined Home Group as a night Support Co-ordinator developing his skills, moving to a Day Support Co-ordinator role and now is managing the service after a completing a secondment as part of our Grow Our Own at another service. He is passionate that with the right support and guidance we can ensure the most vulnerable people in society can receive person centred support, with a firm belief in Housing First. You will join our newly formed ETAAS team supporting customers to rebuild their lives after periods of street homelessness or a breakdown in their current accommodation. Joining Ash who has relocated from Cornwall to join the team, Tracey who has also joined from a different service and joining our Housing Management Team. Our concierge team has grown we have welcomed, Khloe, Emeka, Emenike and Lee joining Ishan who was also part of the previous project.

Job details
• We work with our customers providing 24-hour support, which means you’ll be working on a rota basis. This role is based during the day working alternate weekends.
• Where we’re flexible with you, we do need to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!
• Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
• You’ll need an Enhanced DBS check done and we pay for that.
