The successful applicant will be expected to: Provide daily assessment and management of ICU/HDU patients as directed by the senior clinical team. We routinely manage complex surgical patients, as the hospital is a tertiary referral centre for peritoneal malignancy surgery and resectional hepatobiliary work. We also manage complex cardiology patients, as the hospital is the Trust hub for cardiology, with a 24/7 PPCI service. The Trust is keen to attract the highest calibre of trainee doctors. It realises that to maintain that aim for the future the quality of teaching and training must be of the highest order. Consultant staff are responsible for the training and supervision of all junior medical staff and time is expected to be regularly allocated to their teaching.

Clinical: The post holder will take part in a full shift rota, along with other trainees, which provides 24 hour care for patients including weekends and night shifts. Administrative: Candidates must possess full GMC registration with a license to practise at time of application Research: There are no specific research duties in this post, however there may be opportunities to take part in ongoing clinical trials. Teaching: The department has a strong teaching commitment to junior doctors. There is both formal and informal training as well as educational supervision and guidance for the postholders provided in each attachment. Formal study-leave to attend relevant regional and national courses, (allocation the same as regional trainees) with reimbursement of expenses (according to Trust policy and up to the annual leave limit) also provided. The role will primarily be based in critical care with the option to develop specialist interest skills and be supervised by specialists in the above areas to develop clinical and non-clinical skills managing these patient groups. Clinical The successful candidate will Share the responsibility for the assessment, resuscitation and treatment of all patients referred to the Critical Care Unit, working as part of a team with the on-call ICU consultant. Clinical: The postholder will, together with colleagues, be responsible for the provision of services to the Guys & St Thomas NHS Foundation Trust to include: (a) Diagnosis and treatment of patients of the trust in such hospitals, health centres or clinics or other premises as required. The post holder will be expected to provide high quality care to patients in the Paediatric Intensive Care, and to provide outreach services to children at risk of deterioration in the inpatient paediatric areas at St Marys Hospital. They will also contribute to the research, teaching and training activities on the Intensive Care unit, including providing educational and clinical supervision. The post holder will work with medical, nursing and managerial colleagues to ensure the continued high performance of the intensive care unit and will share responsibility for junior staff management. They will be expected to participate in governance activities and strategy and business planning activities of the department. The post holder will need to demonstrate excellent leadership skills. They will be expected to undertake the administrative duties associated with the care of their patients. The post holder must be available by telephone and be able to attend the hospital within approximately 30 minutes when on call. Continuity is key to safe, effective, and person-centred care, so establishing productive professional relationships with care home staff and management, whilst getting to know the residents over time will support the delivery of excellent care in acute, long-term, and palliative conditions. The Elderly Care team with 14 funded Consultant Geriatrician posts (13 in place) currently provide a wide range of hospital and community based services including Acute Frailty services as well as running specialist clinics for patients with Parkinsons disease, falls and general medical conditions. We have recently started a Rapid Access Clinic for Elderly as part of Frailty- Same day Emergency Care (F-SDEC). Inpatient elderly care wards include 5 acute elderly care wards and an established purpose built Acute Frailty Unit with10 Assessment beds, 22 short stay beds and 2 clinic rooms. In addition to existing services, we are seeking colleagues to take on the challenge of developing new models of care for frail elderly people in the community such as Frailty. Pioneering work has already established Integrated Locality Review meetings (MDT meetings, 4/week). Consultants are working closely with existing community teams to proactively assess and manage frail individuals in the community. We are about to open a 22 bedded Acute Unit based in a Community Hospital in Sheppey. The post holder will join the existing team of full time Consultants. The post holder will need to take responsibility for the management of patients admitted to his/her specialty Elderly Care ward (12-15 patients) routinely and those admitted under their care whilst on call for Acute Frailty. The latter would average 12-15 per on call. These patients are then streamed to Frailty Short Stay Unit if early discharge planned (<48 hours) or one of the 5 Frailty Base wards for longer admissions. Acute Frailty Unit has a dedicated Elderly care Team with 2 Consultant Geriatricians supported by their own team of junior doctors and a Multidisciplinary Team. There are 10 funded posts for Middle Grades in the department with currently 6 in post. Elderly care Consultants participate in an Acute Frailty On call service. The post-holder will be paired with another Consultant so that in a 12 week block they provide 2 weeks of weekday on call and 2 weekends on call. Full registration with the GMC and the ability to make decisions at a senior level, the role will allow you the opportunity to continue your personal development, and participate in the teaching, research and governance programs within the Department. All subspecialist interests are welcome. There is an opportunity, in negotiation with the other consultants, for participation in the running of the established clinics such as Falls and Movement Disorder Clinic. Clinical: The post will attract up to 10 programmed activities (7.0 CHIPS / 3.0 St Oswalds). Within the 10 PA job plan, appropriate time will be built in for SPA activity. Planned activities will include home, hospital and hospice visits, complex case meetings, contributing to specialist team MDTs and developing links within the region; with community teams and inpatient specialist teams.
