JOB DUTIES INCLUDE BUT NOT LIMITED TO:
The Physical Therapist is assigned to the Caregiver Support Program. The primary function of the PT Clinical Assessor is direct patient care, including completing functional assessments to a diverse population in outpatient and community settings within CSP Program of Comprehensive Assistance for Family Caregivers (PCAFC). The incumbent will complete functional assessments of Veteran applicants utilizing a prescribed instrument The Veteran Functional Assessment Instrument (VFAI) and other assessment tools to determine if a Veteran applicant requires personal care services from a caregiver due to inability to perform an activity of daily living (ADL) and/or due to the need for supervision, protection or instruction. In addition to completing the VFAI and other assessments, the incumbent will engage in inter-professional collaboration with: social work, nursing, PT, OT, PACT team, MD, PA, DO, NP, psychologists, counselors and a variety of other medical professionals.

Complete home visits in the community to determine Veterans' and caregivers' needs within the home. The incumbent will support the staffing needs of the medical center and will complete other duties as assigned.

Assists in the day-to-day operations of the Caregiver Support Program specific to his/her area of assignment and ensures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives.

The Clinical Assessor advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues,

Utilize and assist in maintaining a data monitoring system to measure the efficacy of the program and assists in collecting and analyzing local data for reports for: local management, VISN Caregiver Support Program Leads, Centralized Eligibility and Appeal Teams, the National Caregiver Support Program, Care Management and Office of Patient Care Services.

Conducts Veteran, caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues.

Develops and fosters relationships with community partners to assist with system navigation, maintain collaboration and coordinate services Maintains basic knowledge of local VA medical center programs, community services and VA Central Office caregiver support initiatives.

The Clinical Assessor, in collaboration with CSP staff, provides referrals to VA medical and mental health treatment and to other VA services including VA benefits, community-based social services and other non-VA entitlement programs.

Receives the referral and schedules the patient for evaluation and treatment in a timely fashion; chart review will allow the incumbent an opportunity to assess appropriateness of the referral and possible contraindications for treatment.

• Responsible for knowledge of concepts, principles, and practices of Recreation Therapy and the use of recreation modalities to treat the physical, mental, emotional, biopsychosocial, and leisure needs of patients. The Recreation Therapist provides treatment interventions incorporating the four-step process of assessment, planning, implementation, and evaluation of patient treatment services. She/he independently applies evidence-based best practice therapeutic interventions.

• Responsible for completing recreation assessments on veterans, as well as, completing MDS assessments.
• Apply advanced evaluation methodologies and treatment theories in the provision of care to a vast array of patients with differing functional impairments. They provide clinical practice guidance for other recreation and/or creative arts therapy staff.
• Monitor workflow, make work assignments, provide input on performance, resolve daily workplace issues, and maintain efficient flow of patient care and assist with the monthly veteran activities. The lead also works with volunteer services on renewal of certain subscriptions, equipment, snacks etc.
• Coordinate with volunteer services for volunteers and veteran items as it deals with the budget as it relates to recreation and any donated items as it pertains to the veterans. The lead may provide primary reports to the Associate Nurse Executive of CLC/GEC for the daily delivery of department programs, reporting, staffing, and scheduling.
• Provide training for the department's auxiliary staff, volunteers, and trainees and instruction in maintenance of all service equipment and coordinate patient care plans to reduce redundancies and delays in patient treatment.
• Ability to effectively communicate objectives, goals, program, standards, practices, and procedures.
• Ensure reports are monitored, communicate the status progress of work, evaluate program quality to ensure that methods, deadlines, and standards are met, along with prioritizing workload.
• Ability to interpret clinically appropriate data and make recommendations to optimize quality, efficiency, performance, and productivity within services.
• Ability to be responsible for assignment of work to be performed, competency assessment, assist in the selection of staff, recommendation of awards.
• Knowledgeable to follow national and facility guidelines (i.e., SOPs. National directives). The lead will also be part of the interdisciplinary team, and act subject matter expert in recreation, and may act as a liaison between specialty care and other areas that would affect coordination of care, healthcare access, assessment, planning and evaluating future needs.
