RESPONSIBLE FOR:Management and on-going development of the Hospice at HomeTeam including Senior Staff Nurses, Healthcare Assistants and supporting students on placement. An exciting opportunity has arisen to join the Children's Community Health Service as a Safeguarding and Children in Care Lead 0-19.

The Post Holder will be responsible for leading on the delivery of safeguarding to the Universal health service and children’s community health services (0 – 19 yrs), including Health Visiting, Family Nurse Practitioners and School Nursing. The post-holder will be responsible for ensuring patients treatments, assessments (including sample collections), follow-ups and data collection are conducted according to the appropriate research protocols ensuring all clinical research activity is ICH GCP and UK Policy Framework in Health and Social Care compliant. The Role;

As an Activities Coordinator you will take a key role in delivering the provision of activities, focussing on functional daily living skills whilst reducing risk issues. As a Lead Practitioner, you will coordinator care by working closely with people who are suffering from various mental health conditions in their own homes or in the community to help them recover from their illness and improve their general quality of life. This role includes assessing patients, providing care/support, developing risk assessments, collaborative care planning, medication management and physical health monitoring, managing patient documents and all other general nursing duties.

Alongside becoming an advocate for the people in your care, supporting and signposting them, so they can focus on their recovery and helping them met their life goals.

If you are an OT who would like to expand your experience within different mental health settings we currently have a vacancy for a full time Occupational Therapist:

· Within a rotational post you will be based with a variety of teams across the Blackpool, Fylde & Wyre. You will work within each team for 12 months. The teams will include;



- Older Adult Community Mental Health Services

- Adult Community Mental Health Services

- Mental Health In-Patient Wards (Older Adult Dementia)

You will have the opportunity to develop and enhance your skills using preceptorship and will work with and receive supervision from experienced therapists. There are plenty of opportunities for professional development including evidence based best practice forums, AHP journal clubs and an AHP research and innovation forum. We are looking for a therapist who is enthusiastic about working with people with mental health needs.
