The Community Living center (CLC) Nursing Assistants (NAs) are responsibilities include all activities of daily living and general patient care duties to help the healthcare team restore the Veteran to the highest level of independence or remain comfortable toward the end of life. The NA will provide support for diagnostic procedures, nursing treatments, patient charting, and reinforcement of patient education. The performance of these tasks involves direct working relationships with Veterans and participation as a member of the nursing care team in direct service to Veterans.
Nursing Assistant (NA) CLC is a member of the nursing care team and assist licensed nursing staff in the care of patients/residents receiving outpatient care, home, or community living care, or other patients/residents who are in acute, sub-acute or chronic states of illness. The distinguishing factor is that patient/resident assignments typically involve more complex nursing needs which can vary within a range of predictable to unpredictable requirements. The CLC Nursing Assistant at this level also assume more of a peer education/mentorship role by assisting with the coordination of new NA orientation and overseeing/assessing their practical experience while in a clinical setting. They will have the ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. Completed work may require routine or detailed higher-level review depending on the complexity of the duties. Demonstrates ability to properly use equipment, materials, and supplies in simple diagnostic and treatment procedures such as glucometer test for blood glucose level, EKGs, bladder scanning, collection of urine samples, sputum, and stool. Delivers samples to lab and picks up blood products from the blood bank. Safeguards Veteran privacy and maintains confidentiality of all electronic and printed Veteran information according to HIPM guidelines, the Privacy Act, and as stated in the medical center policy on Automated Information Security (AIS). Maintains confidentiality of assigned access code and by exiting (logging out) from the AIS system when leaving terminal. Does not share security codes and does not allow others to use the computer under own security codes. Maintains Veteran confidentiality by not discussing Veteran information except when necessary to authorized staff only in order to meet health care needs. Ensures application of institutional policy to protect information from unauthorized release, loss, alteration, or deletion. Does not use thumb-drives or other means, such as photocopying, to remove Veteran data from the medical center. Position required to maintain a safe NHPPD.
The duties for the Community Living Center Nursing Assistant include but not limited to:
- Plans, organizes, and prioritizes assigned workload.
- Assists providers and nurses in the performance of diagnostic examinations or procedures such as vital sign monitoring, obtaining weight and height, preparing/positioning Veterans.
- Records basic data and patient responses, reporting significant changes to the RN.
- Performs a complete range of patient care related to urinary and bowel elimination, feeding, oral intake, oxygenation, and other duties as assigned by the RN after competency has been demonstrated.
- Collects and labels specimens (urine, stool, and sputum). Delivers specimens to the laboratory section of the facility.
- Assists by positioning and draping as well as emotionally supporting patients/residents during a variety of diagnostic and treatment procedures.
- Measures and records intake and output.
- Able to observe Veterans' physical or emotional status and refer deviations and/or problems to supervisor in a timely manner.
- Maintains a clean, safe, environment of care that is conducive to Veteran privacy and rehabilitation.
- Assists in orienting patients/residents to the outpatient and/or inpatient hospital environment.
- Follows established SOPs and checklists in the use, set up, pre-cleaning, storage, reprocessing, and maintenance of all Reusable Medical Devices, as applicable.
- Meets education requirements for Information Security and Patient Privacy.
Work Schedule: Full time, TOD includes Weekends, Holidays, Evenings and Nights hours and must be flexible to meet needs of organization. TODs are 0600-1630, 1600-0230, 0200-0830, 0730-1800. TOD determined during selection.
