The Department of Medicine at the VA Northeast Ohio Healthcare System is seeking a full-time Nurse Practitioner (NP) interested in an Acute Medicine position working in our Community Living Center (CLC). The CLC is a 174-bed post-acute care facility that provides Veterans with skilled nursing care, skilled physical and/ or occupational rehabilitation, and dementia care. Primary responsibilities include comprehensive medical care of our CLC residents as part of their interdisciplinary team, including but not limited to the following:
- Provide clinical care management to CLC residents through weekly visits/ encounters: perform resident interviews and physical examination, obtain appropriate workup and expert consultations as needed with medical and/ or surgical subspecialists, coordinate care with other specialties and disciplines to elaborate comprehensive disease and medication management care plans, complete history and physical as well as progress notes.
- Provide episodic acute care as dictated by the needs of the Veterans: identify changes in condition, complete assessment, initiate treatment, and initiate transfer and transfer documentation when a Veteran moves to a higher level of care, readmission documentation upon their return, and discharge planning and documentation.
- Recognize and understand resident medical and psychosocial needs and identify resources to meet those needs and achieve desired outcomes pertinent to their CLC stay and post-CLC disposition, in collaboration with mental health specialists, case management and social work, as well as CLC leadership.
- Participate in a cohesive interdisciplinary model of care through collaboration with other CLC disciplines within defined venues such as weekly interdisciplinary team rounds, discharge meetings, bioethics meetings, mental health meetings, wound care meetings etc.
- Educates patients and families on numerous health care topics and issues.
- Identify opportunities for improvement and initiate performance improvement processes using interdisciplinary performance approaches to improve care delivery within the CLC.
- Participate in performance improvement projects as assigned, as well as cover for care planning meetings.
- Assume a leadership role in utilizing problem solving methodologies to resolve system and/or process problems that impact Veteran care, staff performance, and/or systems improvements.
