Salary: £12.00 - £13.50 per hour + Excellent Benefits

The Role:
As a Care Coordinator, you'll be maintaining the efficient operation of home care agency and delivering exceptional care to clients.
Responsibilities:
- Create and sustain client care plans in partnership with clients, families, and healthcare professionals.
- Arrange and manage caregiver visits, ensuring sufficient staffing levels and appropriate skill alignment.
- Oversee client well-being and progress, making necessary adjustments to care plans.
- Perform initial assessments to identify client needs and preferences.
- Consistently communicate with clients and families about care plans, schedules, and any changes.
- Assist in the recruitment, onboarding, and training of new caregivers.
- Keep accurate records of caregiver qualifications, availability, and performance.
Requirements:
Essential:
- Previously worked as a Care Coordinator or in a similar role.
- Possess experience in the home care industry or a related field.
- Strong communication and interpersonal skills.
- Excellent organisational and time management abilities.
- Skilled in Microsoft Office and scheduling software.
Desirable:
- Background in care planning and risk assessments.
- Qualification in health and social care, such as NVQ Level 3.
- Understanding of CQC regulations and standards.
- Valid driving license and access to a vehicle.
