OverviewWorks under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis. The RN may work in a variety of settings (typically in a home care type setting) and roles within Riverside Health System. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. The RN On-Call provides support to the agency after hours via phone or patient visit as necessary. Will take calls from patients regarding issues or concerns and triage the clients needs as to whether a visit is necessary or if the problem can be handled via patient/caregiver education via phone. Will be responsible for taking any after hours referrals and forwarding information to the Care Manager on call. Is responsible for assessing all of the patient's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or via patient visit. The On-Call nurse works effectively within RHCD to maintain an environment of excellence which is patient focused and provides timely, appropriate, compassionate, quality client care. Supports the Hospice Philosophy.

- Takes calls from clients regarding issues or concerns and triages the clients needs as to whether a visit is necessary of the problem can be handled via patient/caregiver education via phone.
- Accurately completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Required documentation is to be completed no later than the last day of the month in order to provide timely billing.
- Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.
- Assesses client's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or patient visit. Demonstrates assessment ability by applying standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops an initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.
- Supervises RNs and LPNs in providing services. Participates in training and support of all team members, volunteers and students as needed. Participates in multi-disciplinary team meetings and discussions regarding quality and outcomes.
- Delegates and prioritizes tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems to meet need.

- Bachelors Degree, Nursing (Preferred)
ExperienceLicenses and Certifications- Registered Nurse (RN) - State Department of Health Professions (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
- Driver's License (Required)
